摘 要:目的探讨ICU中急性心力衰竭合并呼吸衰竭患者采用无创呼吸机治疗的效果。方法选取2018年1月至2019年1月在辽宁省大连市中心医院急诊ICU与神经内科的80例急性心力衰竭合并呼吸衰竭患者,随机分为试验组和对照组,每组40例。两组患者入院后均行常规治疗,试验组给予无创呼吸机辅助呼吸,对照组给予持续低流量吸氧。比较两组患者在治疗后动脉血氧分压(PaO2)、血液pH值,动脉血二氧化碳分压(PaCO2),血氧饱和度(SaO2)及氧合指数(PaO2/FiO2)变化情况,并比较两组患者治疗后24 h的血清氨基末端B型钠尿肽原(NT-pro BNP)及血清中心肌肌钙蛋白Ⅰ(cTnⅠ)水平。结果治疗后,试验组患者血液pH值、PaO2、SaO2及PaO2/FiO2均高于对照组,PaCO2低于对照组,差异均有统计学意义(P<0.05)。在治疗后24 h,试验组血清中NT-pro BNP及cTnⅠ明显低于对照组,差异均有统计学意义(P<0.05)。结论针对急性心力衰竭合并呼吸衰竭患者采用无创呼吸机治疗,可显著改善患者的血气指标,减少血清NT-pro BNP及cTnⅠ含量,提高抢救效果。Objective To investigate the effect of non-invasive ventilator on acute heart failure combined with respiratory failure in ICU.Methods From January 2018 to January 2019,80 patients with acute heart failure complicated with respiratory failure in the Department of Emergency ICU and Neurology of Dalian Central Hospital in Liaoning were selected and randomly divided into experimental group and control group.Patients admitted to the two groups with acute heart failure complicated with respiratory failure received routine treatment,40 patients in the experimental group were given non-invasive ventilator assisted breathing.Control group 40 patients were given continuous low flow oxygen.Compared two groups of patients with acute heart failure combination of respiratory failure after treatment PaO2(arterial blood oxygen partial pressure),blood pH and PaCO2(arterial blood CO2 partial pressure),SaO2(oxygen filling degree)and PaO2/FiO2(oxygenation index).The levels of serum NT-pro BNP and cTnⅠwere compared between the two groups 24 h after treatment.Results After treatment,the blood pH value,PaO2,SaO2 and PaO2/FiO2 of the experimental group were higher than those of the control group,and PaCO2 was lower than the control group,and the difference was statistically significant(P<0.05).At 24 h after treatment,serum NT-pro BNP and cTnⅠin the experimental group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The use of non-invasive ventilator therapy for patients with acute heart failure and respiratory failure can significantly improve the patient's blood gas indicators,reduce serum NT-pro BNP and cTnI levels,and improve the rescue ef fect.
