摘  要:目的探讨能谱CT低辐射剂量在泌尿系CT尿路、肾血管一站式成像技术(CTUA)的临床应用。方法120例血尿待查患者,实验组(A组)60例行泌尿系平扫及对比剂2次团注后行肾动脉期-肾盂期(CTUA)及肾实质-肾盂期扫描,扫描参数应用单能量70 keV,自动毫安,ASIR-V60%重建最佳keV图像;对照组(B组)60例扫描参数120 kV,自动毫安,平扫后单次团注行常规3期扫描。对2组重建图像在肾、尿路、血管显示良好率进行主观评价,并测量2组肾盂排泄期图像中集合系统分各段CT值进行对比,计算各段对比噪声比(CNR),记录A组与B组的平均总有效辐射剂量(ED)进行客观评价。结果A组CTUA期在肾、尿路、血管同时显示良好率明显优于B组CTU期,2组尿路系统显示差异无统计学意义(P>0.05);A组肾盂期集合系统各段CT值高于B组CTU肾盂排泄期,其差异有统计学意义(P<0.05)。集合系统各节段间CNR对比,除个别节段A组与B组间存在差异外,大部分节段A组与B组间差异均无统计学意义(P>0.5)。2组总的有效辐射量,A组平均(12.75±1.16)mSv,B组平均(26.97±1.71)mSv,差异有统计学意义(P<0.05)。结论应用能谱技术的CTUA实现了泌尿系CTU及CTA一站式扫描,直接显示泌尿系解剖及病变与邻近血管间的关系,同时降低患者辐射剂量,减少延时扫描期相,保证了图像质量。Objective To explore the clinical application of energy spectrum CT usinglow radiation dose in urinary CT urine tract and angiography(CTUA)imaging technologies.Methods 120 patients with hematuriawere collected.60 patients in the experimental group(A group)were performed urinary tract scan and two bolus injections of renal artery-renal pelvis(CTUA)and renal parenchyma-renal pelvis scans.Single energy 70 keV,and rear ASIR-V60%were applied for reconstructing the best keV image.60 patients in the control group(B group)were performed the routine mode,120 kV,automatic mA.After a plain scan,a single bolus injection was performed for routine three-phase scanning.Thegood rate of kidney,urinary tract and vasculadisplay of two groups of reconstructed images,Group A CTUA and thegroup B CTUwere evaluated.The collecting system of the two groups were measured and compared with the CT values of each segment.In addition,the contrast noise ratio(CNR)of each segment was calculated.The average total effective radiation dose(ED)in the group ACTUA and thegroup BCTU was recorded.The non-parametric rank sum test was used for thedata of the twogroups.The test level(P<0.05)was considered to havestatistical difference.Results The CTUA phase simultaneously showed kidney,urinary tract,and blood vessels,which was significantly better than the CTU phase in the control group.There was no significant difference in urinary system between the two groups(P>0.05).The CT value of the collecting system in Group A was higher than that in Group B at the excretory phase,and the difference was statistically significant(P<0.05).The CNR of each segment of the aggregation system was compared.Except for the difference in individual segments between the experimental group and the control group,there was no significant statistical difference in most segments between the group Aand thegroup B(P>0.05).The total effective radiation dosage of the two groups was(12.75±1.16)mSv in of group A,and(26.97±1.71)mSv in of group B.The difference was statistically signific
