摘 要:目的:探讨肺表面活性物质联合机械通气对新生儿呼吸窘迫综合征(NRDS)患儿血气分析及氧交换指标的影响。方法:选取91例NRDS患儿,依据抽签法分为对照组(n=45)与观察组(n=46)。对照组予以机械通气治疗,观察组在对照组基础上加用肺表面活性物质治疗,比较两组血气分析指标、氧交换指标及并发症发生情况。结果:治疗后,两组PaO2、pH水平高于治疗前,PaCO2水平低于治疗前,且观察组优于对照组,差异有统计学意义(P<0.05);治疗后,两组PaO2/FiO2、PaO2/PAO2水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率4.35%,低于对照组8.89%,但差异无统计学意义(P>0.05)。结论:NRDS患儿采用肺表面活性物质联合机械通气治疗效果较好,可有效改善患儿血气分析指标,提升其氧交换能力水平,并发症较低。Objective:To investigate the effect of pulmonary surfactant combined with mechanical ventilation on blood gas analysis and oxygen exchange index in children with neonatal respiratory distress syndrome(NRDS).Methods:91 children with NRDS were selected and divided into control group(n=45)and observation group(n=46)according to the method of drawing lots.The control group was treated with mechanical ventilation,and the observation group was treated with pulmonary surfactant on the basis of the control group.Blood gas analysis indexes,oxygen exchange indexes and complications were compared between the two groups.Results:After treatment,the levels of PaO2 and pH in the two groups were higher than those before treatment,and the level of PaCO2 was lower than that before treatment,and the observation group was better than that in the control group,the difference was statistically significant(P<0.05),after treatment,the levels of PaO2/FiO2 and PaO2/PAO2 in the two groups were higher than those before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05),the incidence of complications in the observation group was 4.35%,slightly lower than 8.89%in the control group,the difference was not significant(P>0.05).Conclusion:Pulmonary surfactant combined with mechanical ventilation is effective in the treatment of NRDS children,which can effectively improve the blood gas analysis indexes and enhance the level of oxygen exchange capacity.The complications are low.
