摘 要:目的:观察白藜芦醇对慢性阻塞性肺疾病(COPD)合并肺源性心脏病SD大鼠肺功能及心脏功能结构的影响。方法:将45只成年雄性SD大鼠制造COPD合并肺源性心脏病的病理模型,建模后将45只SD大鼠随机分为白藜芦醇组、糖皮质激素组和对照组。白藜芦醇组给予白藜芦醇灌胃治疗28 d,糖皮质激素组给予布地奈德雾化吸入28 d,对照组给予生理盐水灌胃28 d。对比3组治疗前后肺功能和心脏功能结构变化,及不良反应情况。结果:治疗后,肺顺应性值均降低,第0.3秒用力呼气容积/用力肺活量(FEV0.3/FVC)值均升高;肺顺应性变化中以对照组降低最为显著,糖皮质激素组次之,白藜芦醇组降低幅度最小(F=19.048,P<0.05),FEV0.3/FVC水平变化以白藜芦醇组升高最多,糖皮质激素组次之(F=0.076,P<0.05),对照组无明显变化(t=0.153,P>0.05)。治疗后,3组最大呼气流量水平、最大呼气中段流量水平均降低,以对照组降低最为明显,糖皮质激素组次之,白藜芦醇组降低幅度最小,组间差异有统计学意义(F=17.210、34.836,P<0.05)。治疗后3组左室射血分数(LVEF)升高,左室舒张末期内径(LVEDD)增厚,但LVEF升高以白藜芦醇组升高最明显,糖皮质激素组次之,对照组升高最少(F=56.986,P>0.05);LVEDD增厚中以对照组增厚最显著,糖皮质激素组次之,白藜芦醇组最少,差异均有统计学意义(F=142.442,P<0.05)。治疗后,3组室间隔厚度值、左室后壁厚度值均升高,其中以对照组升高最为明显,糖皮质激素组次之,白藜芦醇组增厚幅度最小,差异均有统计学意义(F=287.401、18.726,P值均<0.05)。治疗后,3组右室(RV)游离壁、左室和室间隔(LV+S)值均增加,且以对照组增加最为显著,糖皮质激素组次之,白藜芦醇组增加最小,差异均有统计学意义(F=3.725、8.355,P值均<0.05)。治疗后3组RV/LV+S值均升高,且以对照组升高最为明显,糖皮质激素组次之,白藜芦醇组升高最小,差异均有统�Objective:To observe the effects of resveratrol on cardiac and pulmonary function in SD rats with chronic obstructive pulmonary disease(COPD)and pulmonary heart disease.Methods:Male SD rats were made into a pathological model of COPD and pulmonary heart disease.After modeling,45 SD rats were randomly divided into a resveratrol group,a hormone group and a control group.Resveratrol group was given intragastric administration of resveratrol for 28 days,hormone group budesonide inhalation 28 days,the control group received normal saline for 28 days.Comparison of cardiac and pulmonary function changes before and after the functional configuration of the three groups.Lung function including lung compliance(CL),the ratio of peak expiratory flow(PEF),maximum mid expiratory flow(MMF),0.3 second forced expiratory volume(FEV 0.3)/forced vital capacity(FVC),cardiac function and structural indicators include left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),interventricular septal thickness(IVST)and posterior left ventricular wall thickness(PWT),and free ventricular(RV)free wall and left ventricular mass and interventricular septum(LV+S)of,and RV/(LV+S)ratio.Results:After treatment,the CL value decreased and the FEV 0.3/FVC value increased.Among the CL changes,the control group had the most significant decrease,followed by the hormone group,and the resveratrol group had the smallest decrease(F=19.048,P<0.05).The level of FEV 0.3/FVC increased most in the resveratrol group,followed by the hormone group(F=0.076,P<0.05),and no significant change in the control group(t=0.153,P>0.05).After treatment,the PEF levels and MMF levels of the three groups decreased,with the control group being the most obvious,followed by the hormone group,and the resveratrol group had the smallest decrease.The difference between the groups was statistically significant(F=17.210,34.836,both P<0.05).After treatment,LVEF increased and LVEDD thickened in the three groups,but the increase in LVEF was most obvious in the
