摘 要:目的:分析厦门市儿童医院住院患儿呼吸道感染病例的病毒病原学特征,为呼吸道病毒感染性疾病的预防控制、诊断、治疗,以及长远监控、研究提供依据。方法:收集2019年1月1日-12月31日厦门市儿童医院住院患儿呼吸道7 758例鼻咽拭子、痰液和肺泡灌洗液标本,采用实时免疫荧光法对呼吸道合胞病毒(RSV)、腺病毒(ADV)、甲型流感(FluA)、乙型流感(FluB)、副流感病毒1、2、3型(PIV 1、2、3型)进行免疫荧光检测,分析上述七项呼吸道病毒的感染率,以及在不同性别、不同年龄组和不同季节感染病例的感染特点。结果:7 758例标本中1 605例检测为阳性,阳性检测率为20.69%。其中RSV占11.47%,ADA占4.69%,FluA占0.70%,FluB占0.37%,PIV 1占0.44%,PIV 2占0.22%和PIV 3占2.80%。4 611例男性患儿中阳性检测率为22.47%,3 147例女性患儿中阳性检测率为18.08%,男女患儿阳性检测率比较,差异有统计学意义(P<0.05)。7 758例患儿不同年龄组之间各种病毒感染总阳性率差异有统计学意义(P<0.05)。呼吸道病毒检测率因季节而异,春、冬两季呼吸道病原体阳性率明显高于夏、秋两季,且差异有统计学意义(P<0.05)。结论:RSV和ADV是2019厦门地区呼吸道病毒感染的主要病原体,不同年龄组之间各种病毒感染率有差异,感染存在季节特征。Objective:To analyze the viral etiological characteristics of respiratory tract infections in children hospitalized in Xiamen Children’s Hospital,and provide evidence for the prevention,control,diagnosis,treatment,long-term monitoring and research of respiratory viral infectious diseases.Method:A total of 7 758 nasopharyngeal swabs,sputum and alveolar lavage fluid specimens of children inpatients in Xiamen Children’s Hospital from January 1 to December 31,2019 were collected,and respiratory syncytial virus (RSV),adenovirus (ADV),influenza A (FluA),influenza B (FluB),parainfluenza virus types 1,2,and 3 (PIV 1,2,and 3) were detected by real-time immunofluorescence,and the above seven items were analyzed,as well as the characteristics of infection in different genders,different age groups,and different seasons.Result:Among 1 605 samples from 7 758 samples,the test was positive,and the positive detection rate was 20.69%.The remaining RSV was 11.47%,ADA was 4.69%,FluA was 0.70%,FluB was 0.37%,PIV 1 was 0.44%,PIV 2 was 0.22% and PIV 3 was 2.80%.The positive detection rate among 4 611 male children was 22.47%,and the positive detection rate among 3 147 female children was 18.08%.The positive detection rate of male and female children was statistically significant (P<0.05).There were statistically significant differences in the total positive rates of various viral infections among 7 758 children with different age groups (P<0.05).The detection rate of respiratory viruses varies in seasons was different.The positive rate of respiratory pathogens in spring and winter was significantly higher than that in summer and autumn,and the difference was statistically significant (P<0.05).Conclusion:RSV and ADV are the main pathogens of respiratory virus infection in Xiamen in 2019.The infection rate of various viruses varies among different age groups,and the infection has seasonal characteristics.
