基于真实世界刺五加注射液治疗6364例高血压患者临床应用特征

摘　　要：目的:基于真实世界数据探讨刺五加注射液临床应用特征,对药物的临床使用及深入分析提供临床指导,提高临床用药的有效性。方法:通过分析20家医院的医院信息系统(HIS)数据库中使用哈尔滨珍宝制药有限公司刺五加注射液的6 364例高血压患者的住院信息,采用计数资料的统计描述、和关联分析等方法。结果:患者年龄最小值为25岁,最大值为102岁,中位数是70岁,均值为67.67岁,65~75岁年龄段内人数最多,所占比例为36.29%,总男女比例为0.832∶1;前15位经中医诊断,肝肾亏虚证记录次数最多,为875次,占15.86%,其次为气虚血瘀证为533次,占9.68%;前15位含有瘀血诊断的中医证候名称有6个,共1 380次,占25.06%,气血阴阳虚证的有8个,共2 171次,占39.41%;痰证有4个,共640次,占11.82%;联合用药药理两项联合分析{祛瘀剂}->{钙通道阻断药}例数最高。结论:刺五加注射液在真实世界中以老年人为主;除肝肾阴虚外,血瘀证存在比例较大,要注重活血化瘀疗法的应用,根据疾病的并发症配合使用治疗心脑血管的药物、抗癫痫药物,同时使用扶正剂、泻下剂、系统性使用抗菌药等辅助治疗。Objective:To explore the clinical application characteristics of Ciwujia Injection based on the real world data,provide clinical guidance for the clinical use and in-depth analysis of drugs,and improve the clinical effectiveness.Methods:The hospital information system(HIS)database of 20 hospitals used Ciwujia injection of Haerbin Zhenbao Pharmaceutical Co,Ltd.was used to analyze the inpatient information of 6364 patients with hypertension,and the statistical description and correlation analysis of count data were used.Results:The minimum age of patients was 25 years old,the maximum was 102 years old,the median was 70 years old,and the average was 67.67 years old.The amount of people in 65~75 years was the most,accounting for 36.29%,and the total ratio of male and female was 0.832∶1;among the top 15 TCM diagnosis,liver and kidney deficiency syndrome recorded the most(875 times),accounting for 15.86%,followed by qi deficiency and blood stasis syndrome(533 times),accounting for 9.68%;among the top 15 TCM syndrome names,there were 6 ones containing blood stasis diagnosis,1380 times in total,accounting for 25.06%;8 cases of Qi blood Yin Yang deficiency syndrome,a total of 2171 times,accounting for 39.41%;4 cases of phlegm syndrome,a total of 640 times,accounting for 11.82%;the number of cases of{blood stasis agent}->{calcium channel blocker}was the highest.Conclusion:Ciwujia Injection in the real world is mainly used for elderly;except for liver and kidney yin deficiency,there is a large proportion of blood stasis syndrome.We should pay attention to the application of blood activating and stasis removing therapy,and use drugs for cardiovascular and cerebrovascular diseases and antiepileptic drugs according to the complications of the disease,and at the same time,usestrengthening agents,purgative agents,systemic antibiotics and other auxiliary treatment.
