超声引导下双侧颈浅丛及单侧颈深丛阻滞复合静脉麻醉在甲状腺手术中的应用研究

摘　　要：目的探究甲状腺术中超声引导患侧颈深丛+双侧颈浅丛阻滞麻醉的应用效果。方法选取2019年1月~2020年6月江西省赣州市立医院收治的60例甲状腺手术患者,用计算机随机分组法分为A组、B组、C组,每组各20例。A组行超声引导患侧颈深丛+双侧颈浅丛阻滞麻醉,B组行单纯颈丛阻滞,C组行全麻插管,比较三组的平均动脉压及心率变化、并发症发生率、麻醉舒适度评分。结果A组不同时段的平均动脉压及心率优于B组与C组,差异有统计学意义(P<0.05);A组的并发症总发生率低于B组与C组,差异有统计学意义(P<0.05);A组的麻醉舒适度评分高于B组与C组,差异有统计学意义(P<0.05)。结论在甲状腺术中,超声引导下患侧颈深丛+双侧颈浅丛阻滞麻醉,具有显著的麻醉效果,值得临床借鉴。Objective To explore the application effect of ultrasound-guided deep cervical plexus+bilateral superficial cervical plexus block anesthesia in thyroid surgery.Methods A total of 60 cases of thyroid surgery admitted to Ganzhou Municipal Hospital,Jiangxi Province from January 2019 to June 2020 were selected as research subjects.The computer randomized grouping method was used to divide them into three groups,namely group A,group B,and group C,20 cases in each group.Group A underwent ultrasound-guided deep cervical plexus+bilateral superficial cervical plexus block anesthesia,group B underwent simple cervical plexus block,and group C underwent general anesthesia intubation.The mean arterial pressure and heart rate changes and complications of the three groups were compared.Incidence rate,anesthesia comfort score were compared.Results The mean arterial pressure and heart rate in different time periods in group A were better than those in group B and group C(P<0.05);the total incidence of complications in group A was lower than that in group B and group C(P<0.05);comfort score of group A was higher than that of group B and group C,the differences were statistically significant(P<0.05).Conclusion In thyroid surgery,ultrasound-guided deep cervical plexus+bilateral superficial cervical plexus block anesthesia has a significant anesthetic effect and is worthy of clinical reference.
