摘 要:目的:探究双节段人工颈椎间盘置换术治疗颈椎病后颈椎矢状位参数的变化。方法:回顾性分析2014年10月至2019年7月行双节段人工颈椎间盘置换术(cTDR)患者48例。测量患者术前和末次随访时颈椎侧位X线片的矢状位参数,包括手术节段角度(SA)、C2~C7 Cobb角和矢状位轴向距离(SVA)及T1倾斜角(T1S)。用配对样本t检验比较同组内手术前后各参数是否有差异,用Pearson相关性分析患者手术前后影像学参数间的相关性。结果:cTDR术后末次随访时SA、C2~C7 Cobb角、SVA值及T1S与术前相比有所增加,差异有统计学意义(P<0.05)。影像学参数Pearson分析结果示:T1S及C2~C7 Cobb角与SA相互呈正相关,差异有统计学意义(P<0.05)。结论:双节段人工颈椎间盘置换术可恢复颈椎活动范围,改善颈椎矢状位参数。Objective:To investigate the changes of sagittal parameters of cervical vertebra after double cervical artificial intervertebral disc replacement for cervical spondylosis.Methods:A retrospective study of 48 cases of cervical total disc replacement(cTDR) from October 2014 to July 2019 was carried out.Sagittal parameters of cervical lateral X-ray films were measured before and at the last follow-up,including surgical angle(SA),C2~C7 Cobb angle,C2~C7 sagittal vertical axial distance(SVA) and T1 slope(T1S).Paired sample t test was used to compare the difference of parameters before and after operation in the same group.Pearson correlation was used to analyze the correlation between imaging parameters before and after operation.Results:At the last follow-up after cTDR,SA,C2~C7 Cobb angle,C2~C7 SVA and T1S were significantly increased(P<0.05).Pearson analysis of imaging parameters showed that T1S and C2~C7 Cobb angle were positively correlated with SA(P<0.05).Conclusion:Double-segment cervical disc replacement can restore the range of motion of the cervical spine and improve the sagittal parameters of the cervical spine.
