动态心电图缺血性J波在急性冠脉综合征的诊断价值分析

摘　　要：目的:研究动态心电图缺血性J波在急性冠脉综合征的诊断价值。方法:回顾分析2018年1月-2019年12月在笔者所在医院确诊的100例急性冠脉综合征患者,其中不稳定心绞痛和急性心肌梗死患者各50例,提取无症状及有疾病发作24 h动态心电图检查结果各1次,对动态心电图缺血性J波进行临床分析。结果:不稳定心绞痛和急性心肌梗死患者有症状发作时24 h动态心电图缺血性J波发生率(60.0%、68.0%)均高于无症状发作时(12.0%、16.0%),差异均有统计学意义(P<0.05);在有症状发作24 h动态心电图出现缺血性J波患者中,30例不稳定性心绞痛患者中有25例患者出现室性心律失常,发生率为83.3%;34例急性心肌梗死患者中有28例患者出现室性心律失常,发生率为82.4%。结论:动态心电图缺血性J波对急性冠脉综合征有着较好的诊断价值,也有助于指导医师对急性冠脉综合征发生室性心律失常的预防和治疗,提高临床诊疗水平,具有较高的临床价值。Objective:To study the diagnostic value of dynamic electrocardiogram ischemic J wave in acute coronary syndrome.Method:A retrospective analysis was performed on 100 patients with acute coronary syndrome diagnosed in our hospital from January 2018 to December 2019,including 50 patients with unstable angina pectoris and 50 patients with acute myocardial infarction,the results of 24 h dynamic electrocardiogram(DCG)with no symptoms and onset of disease were extracted once each,the clinical analysis of ischemic J wave in dynamic electrocardiogram was carried out.Result:The incidence of 24 h DCG ischemic J wave in patients with unstable angina pectoris and acute myocardial infarction at symptomatic attack(60.0%,68.0%)was higher than that at asymptomatic attack(12.0%,16.0%),the difference was statistically significant(P<0.05).In patients with ischemic J wave on 24 h DCG with symptomatic onset,ventricular arrhythmia occurred in 25 cases of the 30 patients with unstable angina,with an incidence of 83.3%,ventricular arrhythmia occurred in 28 of 34 patients with acute myocardial infarction(AMI),with an incidence of 82.4%.Conclusion:Ischemic J wave of dynamic electrocardiogram has a good diagnostic value for acute coronary syndrome,and is also helpful to guide physicians in the prevention and treatment of ventricular arrhythmia caused by acute coronary syndrome and improve the level of clinical diagnosis and treatment,which has a high clinical value.
