膝关节腔注射富血小板血浆对老年患者膝关节骨性关节炎疼痛的影响

摘　　要：目的探讨膝关节腔注射富血小板血浆(PRP)治疗老年患者膝关节骨性关节炎疼痛的临床疗效。方法选择膝关节骨性关节炎患者80例,男35例,女45例,年龄65~80岁,Kellgren-Lawrence分级Ⅰ—Ⅲ级,随机均分为两组:PRP组和透明质酸组(HA组)。分别记录治疗前和治疗后4、12、48周的膝关节功能(WOMAC评分和Lequesne指数)、汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分,观察注射部位的不良反应和并发症。结果与治疗前比较,两组治疗后4、12、48周的WOMAC评分和Lequesne指数、HAMA评分和HAMD评分均明显下降(P<0.001)。PRP组治疗后48周的WOMAC评分和Lequesne指数、HAMA评分和HAMD评分明显低于HA组(P<0.001)。两组均无出血、血肿和注射部位感染等不良反应和并发症。结论膝关节腔内注射富血小板血浆的疼痛治疗效果优于透明质酸。Objective To investigate the clinical efficacy of intia-articular injection of platelet-rich plasma(PRP)in the treatment of knee osteoarthritis pain in the elderly patients.Methods Eighty patients with knee osteoarthritis,35 males and 45 females,aged 65-80 years,Kellgren-Lawrence gradeⅠ-Ⅲwere randomly divided into two groups:platelet-rich plasma group(group PRP)and hyaluronic acid group(group HA).Knee joint function(WOMAC score and Lequesne index)was used to record knee joint function before treatment and 4,12,and 48 weeks after treatment.Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)were used to evaluate the anxiety and depression of patients before and 4 weeks,12 weeks,and 48 weeks after treatment.The adverse reactions and complications of the injection site were observed.Results Compared with pre-treatment,WOMAC score,Lequesne index,HAMA score and HAMD score decreased 4,12,and 48 weeks after treatment in both groups(P<0.001).Compared with group HA,WOMAC score,Lequesne index,and HAMA and HAMD scores in group PRP decreased significantly 48 weeks after treatment(P<0.001).Conclusion Intra-articular injection of platelet-rich plasma is more effective than intra-articular hyaluronic acid injection.
