PRP联合膝关节周围痛点注射治疗膝骨关节炎疗效评价

摘　　要：目的:评价富血小板血浆(Platelet-Rich Plasma,PRP)联合膝周痛点注射和透明质酸钠(Sodium Hyaluronate,HA)联合膝周痛点注射治疗膝骨关节炎(Knee Osteoarthritis,KOA)的临床疗效。方法:48例KOA患者分成PRP联合膝周痛点注射(观察组)和HA联合膝周痛点注射(对照组),各24例,采用NRS和WOMAC评分评估术前和术后1个月、3个月和6个月的临床疗效。结果:观察组患者随访6个月的有效率为82.1%,而对照组为75.3%。二组的NRS和WOMAC评分较治疗前比较均有统计学差异(P<0.05),且观察组在术后6个月的NRS和WOMAC均优于对照组(P<0.05)。结论:观察组的PRP联合膝周痛点注射和对照组的HA联合膝周痛点注射治疗KOA均可缓解膝关节疼痛和改善关节功能,而PRP联合膝周痛点注射的远期疗效更优。Objective:To evaluate the clinical efficacy of platelet-rich plasma(PRP)or sodium hyaluronate(HA)intra-articular combined with extra-articular trigger point injection in the treatment of knee osteoarthritis(KOA).Methods:Forty-eight patients with KOA were selected and divided into PRP(experimental group,n=24)or HA(control group,n=24)intra-articular combined with extra-articular trigger point injection.NRS and WOMAC scores were evaluated before,one month,three months and six months after treatment.Results:The effective rate was 82.1%in the PRP group and 75.3%in the HA group during six months of followup.Both NRS and WOMAC showed significant differences after treatment in comparison with pre-operation(P<0.05),and the NRS and WOMAC were better in the PRP group than in the HA group during six months of treatment(P<0.05).Conclusion:PRP or HA intra-articular combined with extra-articular trigger point injection in the treatment of KOA can relieve joint pain and improve function,while PRP combined with trigger point injections had better long-term outcomes.
