摘 要:目的探讨白细胞介素-8(IL-8)基因多态性对α-2b干扰素治疗毛细支气管炎疗效的影响。方法选取我院2018年6月至2020年6月收治的136例毛细支气管炎患儿作为病例组,以同期进行健康体检的136例婴幼儿作为对照组。患儿均采取α-2b干扰素雾化吸入治疗。两组研究对象均进行IL-8基因多态性检测,比较不同基因型患儿的总有效率、症状消失时间、血清人嗜酸粒细胞阳离子蛋白(ECP)、白细胞介素-4(IL-4)、γ干扰素(IFN-γ)水平及不良反应发生率。结果病例组中AA基因型占比、A等位基因频率明显高于对照组(P<0.05)。136例患儿总有效率为80.88%(110/136),其中AT、TT基因型患儿总有效率高于AA患儿(P<0.05),AT和TT基因型患儿总有效率差异无统计学意义(P>0.05)。AT和TT患儿的咳嗽消失时间、喘息消失时间、哮鸣音消失时间、体温恢复正常时间、住院时间明显短于AA患儿,同时TT患儿短于AT患儿,差异均有统计学意义(P<0.05)。治疗后,不同基因型患儿血清ECP、IL-4均较治疗前降低、血清IFN-γ较治疗前升高(P<0.05)。AT和TT患儿治疗后的血清ECP、IL-4低于AA患儿,同时TT患儿低于AT患儿;AT和TT患儿治疗后的血清IFN-γ高于AA患儿,同时TT患儿高于AT患儿(P<0.05)。136例患儿不良反应发生率为13.97%(19/136)。AA、AT、TT基因型患儿不良反应发生率比较,差异无统计学意义(P>0.05)。结论重组干扰素α-2b雾化治疗毛细支气管炎可有效抑制血清ECP、IL-4,上调血清IFN-γ,且安全性高。此外,其对患儿的治疗效果会受到IL-8基因多态性影响,携带A基因对治疗效果有不利影响。Objective To investigate the effect of interleukin-8(IL-8)gene polymorphism on the efficacy of interferon α-2b in the treatment of bronchiolitis.Methods Totally 136 pediatric patients with bronchiolitis admitted to our hospital from June 2018 to June 2020 were selected as case group,and the infants and children who underwent a physical examination during the same period were selected as control group.All the children were treated with aerosol inhalation of interferon α-2b,and were tested for IL-8 gene polymorphism,and the total effective rate,time of symptoms disappearance,serum human eosinophil cationic protein(ECP),and interleukin-4(IL-4),interferon-γ(IFN-γ)levels and incidence of adverse reactions were compared among the children with different genotypes.Results The proportion of AA genotype and the frequency of A allele in case group were significantly higher than those in control group(P<0.05).The total effective rate in case group was 80.88%(110/136).Among them,the total effective rate of patients with AT and TT genotype was higher than that of patients with AA(P<0.05).There was no statistical difference in total effective rate between the patients with AT genotype and those with TT genotype(P>0.05).The cough disappearance time,wheezing disappearance time,time of body temperature returning to normal,and hospitalization time of patients with AT and TT were significantly shorter than those of patients with AA(P<0.05),and it was shorter in TT than in AT(P<0.05).After treatment,the serum ECP and IL-4 of patients with different genotypes were decreased(P<0.05),serum IFN-γ was increased(P<0.05).After treatment,the serum ECP and IL-4 of patients with AT and TT were lower than those of patients with AA(P<0.05),while patients with TT were lower than those with AT(P<0.05);patients with AT and TT had higher serum IFN-γ than those with AA(P<0.05),and patients with TT were higher than those with AT(P<0.05).The incidence rate of adverse reactions in 136 patients was 13.97%(19/136).There was no significant differenc
