摘　　要：目的探讨尺骨近端带钩锁定钢板内固定治疗外踝撕脱性骨折临床疗效。方法将2018年10月—2020年6月,采用尺骨近端带钩锁定钢板内固定治疗的29例外踝撕脱性骨折,总结临床疗效。结果29例均顺利完成手术并获得随访,随访时间3~10个月,平均(5.2±1.0)个月。术后1例踝关节疼痛,未出现内固定失效等并发症。末次随访时按AOFAS踝-后足评分标准评分:优10例,良18例,可1例,优良率96.55%。结论尺骨近端带钩锁定钢板内固定治疗外踝撕脱性骨折具有操作简单,易于掌握,强度大,体积小、厚度薄、远端钩能有效固定外踝较小骨折块,能对抗外侧副韧带牵拉力,无需石膏外固定,允许早期无负重主动行踝关节伸屈锻炼,促进骨性愈合,增强踝关节稳定性,减少创伤性关节炎等并发症少,术后疗效显著等优点。Objective To investigate the clinical effect of proximal ulna hook locking plate internal fixation in the treatment of lateral malleolus avulsion fracture.Methods From October 2018 to June 2020,29 cases of ankle avulsion fractures were treated with proximal ulna hook locking plate internal fixation,and the clinical efficacy was summarized.Results All patients were followed up for 3 to 10 months,with an average of(5.2±1.0)months.One patient had ankle pain after operation,and no internal fixation failure occurred.According to AOFAS ankle hindfoot score,10 cases were excellent,18 cases were good,and 1 case was fair.The excellent and good rate was 96.55%.Conclusion The treatment of avulsion fracture of lateral malleolus with hook locking plate internal fixation is simple,easy to master,large in strength,small in size and thin in thickness.The distal hook can effectively fix the small fracture block of the lateral malleolus,resist the traction of lateral collateral ligament,and do not need plaster external fixation.It allows early weight-free active ankle joint extension and flexion exercise to promote bone healing and enhance ankle joint stability.It has the advantages of less complications such as reducing traumatic arthritis and remarkable curative effect after operation.
