摘 要:目的探讨咳嗽变异性哮喘(CVA)患儿血清神经生长因子(NGF)、神经激肽A(NKA)、P物质(SP)的表达与辅助T淋巴细胞(Th1/Th2)及其细胞因子的相关性。方法选取80例CVA患儿为研究组,健康体检正常儿童80例为对照组。采用雾化吸入3%的高渗盐水及沙丁胺醇400μg收集支气管诱导痰,检测诱导痰炎性细胞计数;采用酶联免疫吸附法(ELISA)检测血清NGF、NKA、SP、神经源性相关炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-5(IL-5)、基质金属蛋白酶-2(MMP-2)、MMP-9]及相关细胞因子[IL-2、IL-4、IL-10、γ干扰素(IFN-γ)]水平;采用流式细胞术检测外周血Th1、Th2细胞水平。采用Spearman检验分析NGF、NKA、SP与Th1/Th2、IL-2、IL-4、IL-10、IFN-γ的相关性。结果研究组巨噬细胞、中性粒细胞、淋巴细胞数、Th1细胞数、Th1/Th2值及IL-2、IFN-γ水平均显著低于对照组(P<0.01),嗜酸性粒细胞、细胞总数、Th2细胞数及NGF、NKA、SP、TNF-α、IL-5、MMP-2、MMP-9、IL-4、IL-10水平均显著高于对照组(P<0.01)。相关性分析结果显示,NGF、NKA及SP与Th1/Th2(r=-0.354、-0.457、-0.502,均P<0.01)、IL-2(r=-0.493、-0.398、-0.376,均P<0.01)、IFN-γ(r=-0.506、-0.385、-0.489,均P<0.01)呈显著负相关,与IL-4(r=0.437、0.382、0.455,均P<0.01)、IL-10(r=0.396、0.508、0.449,均P<0.01)呈显著正相关。结论CVA患儿存在明显的Th1/Th2细胞及其相关细胞因子失衡,炎症因子水平急增,通过NGF、NKA及SP检测可有助于了解CVA患儿病情变化及免疫功能失衡情况。Objective To investigate the relationships of serum nerve growth factor(NGF),neurokinin A(NKA)and substance P(SP)expression to Th1/Th2 cells and their cytokines in children with cough variant asthma(CVA).Methods Eighty CVA children were selected as the study group,and 80 healthy subjects as the control group.Sputum was induced by aerosol inhalation of 3%hypertonic saline and 400μg salbutamol,and the number of inflammatory cells in the induced sputum was measured.Serum concentrations of NGF,NKA,SP,tumor necrosis factor(TNF-α),interleukin 5(IL-5),matrix metalloproteinase-2(MMP-2),matrix metalloproteinase-9(MMP-9),interleukin 2(IL-2),interleukin 4(IL-4),interleukin 10(IL-10)and interferon gamma(IFN-γ)were determined by ELISA.The levels of Th1/Th2 cells in peripheral blood were detected by flow cytometry.The correlations between NGF,NKA and SP expression and Th1/Th2,IL-2,IL-4,IL-10 and IFN-γlevels was analyzed by Spearman correlation analysis.Results The numbers of macrophages,neutrophils,lymphocytes and Th1 cells,ratio of Th1/Th2,and levels of IL-2 and IFN-γin study group were lower than those in control group(P<0.01).However,the number of eosinophils,total number of cells,number of Th2 cells,and levels of NGF,NKA,SP,TNF-α,IL-5,MMP-2,MMP-9,IL-4 and IL-10 in study group were higher than those in control group(P<0.01).Correlation analysis results showed that NGF,NKA and SP were negatively correlated with Th1/Th2(r=-0.354,-0.457 and-0.502,respectively;P<0.01),IL-2(r=-0.493,-0.398 and-0.376,respectively;P<0.01)and IFN-γ(r=-0.506,-0.385 and-0.489,respectively;P<0.01),but positively correlated with IL-4(r=0.437,0.382 and 0.455,respectively;P<0.01)and IL-10(r=0.396,0.508 and 0.449,respectively;P<0.01).Conclusion Children with CVA have obvious imbalance of Th1/Th2 cells and related cytokines,and the level of inflammatory factors increases rapidly.The detection of NGF,NKA and SP can help to understand the changes of the condition of children with CVA and the imbalance of immune function.
