针刺联合定眩止晕汤治疗椎动脉型颈椎病临床研究

摘　　要：目的:观察针刺联合定眩止晕汤治疗椎动脉型颈椎病的临床疗效。方法:选取98例椎动脉型颈椎病患者,按治疗方法不同分为研究组和对照组各49例。对照组给予定眩止晕汤治疗,研究组在对照组基础上联合针刺治疗,14 d为1个疗程,治疗2个疗程。比较2组临床疗效,以及治疗前后基底动脉搏动指数(PI)、阻力指数(RI),颈动脉收缩期最大流速(PSV)、时间平均血流速度(TMFV),基底动脉、椎动脉平均血流速度。结果:研究组总有效率为97.96%,高于对照组的79.59%,差异有统计学意义(P<0.05)。治疗后,2组TMFV及PSV均增大(P<0.05),PI及RI均降低(P<0.05);研究组TMFV及PSV均大于对照组(P<0.05),PI及RI均小于对照组(P<0.05)。治疗后,2组椎动脉及基底动脉平均血流速度均较治疗前增大(P<0.05),研究组椎动脉及基底动脉平均血流速度均大于对照组(P<0.05)。结论:针刺联合定眩止晕汤治疗椎动脉型颈椎病,可加快血流速度,改善临床症状,提高临床疗效。Objective: To observe the clinical effect of acupuncture combined with Dingxuan Zhiyun tang for cervical spondylosis of vertebral artery type. Methods:A total of 98 cases of patients with cervical spondylosis of vertebral artery type were divided into the control group and the study group according to different treatment methods,49 cases in each group.The control group was treated with Dingxuan Zhiyun tang, and the study group was additionally treated with acupuncture based on the treatment of the control group. Both groups were treated for two courses, 14 days being one course. The pulsatility index(PI),resistance index(RI),peak systolic velocity(PSV) and temporal mean flow velocity(TMFV) in basilar artery as well as mean velocity in basilar artery and vertebral artery before and after treatment, and the clinical effect in the two groups were compared. Results:The total effective rate was 97.96% in the study group,higher than that of 79.59% in the control group,the difference being significant(P<0.05). After treatment,TMFV and PSV in the two groups were increased(P<0.05),and PI and RI were decreased(P<0.05);TMFV and PSV in the study group were higher than those in the control group(P<0.05),and PI and RI were lower(P<0.05). After treatment,the mean velocity in basilar artery and vertebral artery in the two groups were increased when compared with those before treatment(P<0.05),and the mean velocity in basilar artery and vertebral artery in the study group were higher than those in the control group(P<0.05). Conclusion: Acupuncture combined with Dingxuan Zhiyun tang for cervical spondylosis of vertebral artery type can speed up blood flow velocity,and improve clinical symptoms and clinical effect.
