中医辨证施护治疗高血压病脑出血临床研究

摘　　要：目的:观察中医辨证施护治疗高血压病脑出血的临床疗效。方法:将高血压病脑出血患者130例采用信封法随机分为2组,对照组65例采用西医常规治疗及常规护理干预,研究组65例在对照组的基础上加用中医辨证施护。2组均治疗随访3个月,比较2组中医证候疗效、血肿吸收、神经功能及肢体功能恢复程度。结果:中医证候疗效总有效率研究组为87.69%,对照组为66.15%,2组比较,差异有统计学意义(P<0.05)。治疗7 d、治疗14 d,2组患者血肿体积均较治疗前明显缩小(P<0.05),且研究组血肿体积明显小于对照组(P<0.05)。治疗后,2组神经功能缺损评分较治疗前降低(P<0.05),上、下肢简式Fugl-Meyer运动量表评分均较治疗前升高(P<0.05);且研究组上述各项评分改善较对照组更显著(P<0.05)。结论:在西医常规治疗及护理干预的基础上,对高血压病脑出血患者加用中医辨证施护,可促进患者血肿吸收,改善中医证候,有助于短期内恢复患者神经功能和肢体功能。Objective:To observe the clinical effect of nursing based on syndrome differentiation of Chinese medicine for patients with hypertensive intracerebral hemorrhage.Methods:A total of 130 cases of patients with hypertensive intracerebral hemorrhage were divided into two groups based on the envelope method,65 cases in each group.The control group was given routine western medicine and nursing intervention,and the study group was additionally given nursing based on syndrome differentiation of Chinese medicine based on the treatment of the control group.The treatment and the follow-up lasted for three months in the two groups.The curative effect of Chinese medicine syndrome,hematoma absorption,and the recovery degree of the nerve function and the limb function were compared between the two groups.Results:The total effective rate of the curative effect of Chinese medicine syndrome was 87.69%in the study group,and66.15%in the control group,the difference being significant(P<0.05).On the 7 thand 14 thdays of treatment,the volume of hematoma in the two groups was significantly decreased when compared with that before treatment(P<0.05),and the volume in the study group was significantly smaller than that in the control group(P<0.05).After treatment,the scores of neurological impairment in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of Simplified Fugl-Meyer Assessment of the upper and lower limbs were increased when compared with those before treatment(P<0.05);the improvement on each score above in the study group was more significant than that in the control group(P<0.05).Conclusion:Based on routine western medicine and nursing intervention,the additional nursing based on syndrome differentiation of Chinese medicine in the treatment of patients with hypertensive intracerebral hemorrhage can promote hematoma absorption,and improve Chinese medicine syndromes.Besides,it can conduce to recovering the nerve function and the limb function in short term.
