自拟化痰逐瘀降压方加减治疗痰瘀互阻型高血压的临床观察

摘　　要：目的研究针对痰瘀互阻型高血压患者采取自拟化痰逐瘀降压方加减治疗方案的价值。方法选取南通市中医院2018年1月至2020年1月符合标准的78例痰瘀互阻型高血压门诊患者开展研究。依据随机数字表法选出39例患者单纯予以苯磺酸氨氯地平片(压氏达)口服治疗(A组),另外39例患者加用自拟化痰逐瘀降压方开展治疗(B组)。对两组患者临床疗效予以评估,观察其收缩压(SBP)以及舒张压(DBP)等血压指标改善情况,并对比治疗前后中医症候积分,统计药物不良反应发生情况。结果B组治疗总有效率为94.87%,显著高于A组的76.92%;B组患者经过临床治疗后,其头晕、心慌、肢体麻木、胸闷痰多及局部胀痛等中医症候积分均优于A组;B组患者SBP及DBP血压指标控制效果优于A组,差异均有统计学意义(P<0.05);两组不良反应发生率相比差异无统计学意义(P>0.05)。结论自拟化痰逐瘀降压方加减治疗方案在痰瘀互阻型高血压患者治疗中合理应用,可显著提升临床疗效,改善患者SBP及DBP水平,降低其中医症候积分,且不良反应较少,安全可靠。Objective To study the therapeutic value of self-made Huatan Zhuyu Jiangya Decoction in patients with hypertension of phlegm and blood stasis resistance type.Methods From January 2018 to January 2020,78 eligible out-patients with sputum and blood stasis type hypertension in Nantong Hospital of Traditional Chinese Medicine were selected for the study.According to the random number table method,39 patients were randomly selected to receive oral treatment with amlodipine besylate tablets(piperidazole)alone(group A),and another 39 patients were added with Huatan Zhuyu Jiangya Decoction(group B).The clinical efficacy of the two groups of patients was evaluated to observe the improvement of blood pressure indexes such as systolic blood pressure(SBP)and diastolic blood pressure(DBP),and the TCM syndrome scores were compared before and after treatment,and the occurrence of adverse drug reactions was counted.Results The total effective rate of group B was 94.87%,significantly higher than that of group A(76.92%);after clinical treatment,the TCM syndrome scores of dizziness,palpitation,limb numbness,chest tightness,phlegm and local distension pain in group B were better than those in group A;the control effect of SBP and DBP blood pressure indexes in group B was better than that in group A(P<0.05);the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion The reasonable application of Huatan Zhuyu Jiangya Decoction in the treatment of hypertension patients with phlegm and blood stasis resistance can significantly improve the clinical efficacy,improve the levels of SBP and DBP,and reduce the TCM syndrome score,with less adverse reactions,which is safe and reliable.
