第1足趾甲皮瓣与第2足趾复合组织组合移植急诊再造拇指的临床应用

摘　　要：目的:报道应用第1足趾甲皮瓣与第2足趾复合组织组合移植急诊再造拇指的临床效果.方法:2013年1月-2018年12月采用第1足趾甲皮瓣与第2足趾骨关节肌腱的复合组织组合移植急诊再造拇指治疗拇指Ⅲ-Ⅳ度缺损10例.结果:术后10例再造拇指全部成活.术后随访3-12月,再造拇指外形与健侧相近,功能恢复满意,指端两点分辨率为5-10 mm.按中华医学会手外科学会上肢部分功能评定试用标准评定:优7例,良3例.供足行走正常,无跛行、疼痛等后遗症.结论:应用第1足趾甲皮瓣与第2足趾复合组织组合移植急诊再造拇指治疗拇指Ⅲ-Ⅳ度缺损,可获得良好的修复外形和满意的功能恢复.Objective:To report the clinical effect of emergency thumb reconstruction by the big toe wrap-around flap combined with the second toe tissue flap transplantation.Methods:From January 2013 to December 2018,10 cases with III-IV degree thumb defect were reconstructed in emergency by the big toe wrap-around flap combined with the second toe tissue flap transplantation.Results:All thumbs in 10 cases reconstructed by combined transplantation survived successfully.The patients were followed up for 3 months to 1 year postoperatively.The reconstructed thumbs had near normal appearance and satisfactory function.The sensation reached almost normal levels,and the two—point discrimination ranged from 5 mm and 10 mm.According to the Chinese Medical Association of Hand Surgery Society reconstruction of thumb and finger function evaluation standard:7 cases were excellent,3 cases were good.The patients were able to weight-bearing walking after operation,no case had obvious pain and claudication.Conclusion:The reconstructed thumbs by the big toe wrap-around flap combined with the second toe tissue flap transplantation had near normal appearance and satisfactory function.
