摘 要:目的建立ABO血型基因的PCR检测方法,评价常规ABO血清学正定法用于新生儿ABO定型的可靠性,探讨血型血清学结合PCR-SSP基因分型法在新生儿ABO血型鉴定中的价值。方法采用血清学方法鉴定110例新生儿血型,结合PCR-SSP基因分型方法做检测,并对其中4例PCR-SSP方法未明确血型的通过测序确定ABO基因型。结果110例新生儿ABO血型标本,血清学检测方法中,2例抗原强度较弱,无法诊断血型,通过PCR-SSP方法诊断为AB型;PCR-SSP检测方法中,106例确定基因型,PCR-SSP方法未明确血型的4例标本通过测序确定基因型,所有基因分型结果与血清学检测结果一致率为100%;新生儿ABO血型正反定型不符率为63%,A、B、O型正反定不符率比较,P>0.05。结论ABO血型基因分型结合血清学分型可用于新生儿ABO血型鉴定,一定程度上可弥补血清学方法的不足。Objective To establish the molecular genetic blood group typing based on PCR-SSP,so as to assess the reliability of direct antigendetermiantson RBCs for the newborns,and discuss the value of the serological typing combined with PCR-SSP for ABO typing in neonates.Methods The blood groups of 110 neonates were identified by serological typing combined with PCR-SSP.The 4 unspecified cases by PCR-SSP were further validated by gene sequencing.Results Among 110 neonatal ABO blood group samples,108 were identified by serological typing,and 2 were successfully confirmed as group AB by PCR-SSP due to the weakened antigen expression.106 cases were genotyped by PCR-SSP,and 4 equivocal cases were further confirmed by gene sequencing.The geno typing results were in full concordance with serological phenotypes.The incidence of discrepancies between direct and indirectantigen determinantsinneonates was 63%,showing no significant differences in the incidences among group A,B and O(P>0.05).Conclusion ABO blood group genotyping combined with serological test can be used for neonatal ABO blood group identification,and can make up the deficiency of serological method to some extent.
