摘　　要：目的消除自身免疫性溶血性贫血(AIHA)对患者Rh血型鉴定及抗体筛查的干扰因素,确保AIHA患者安全、有效的输血。方法将本院血液科2019年1月—2019年8月12名确诊的AIHA患者为研究对象,分别做氯喹放散试验前后的Rh血型鉴定,乙醚放散试验后的自身抗体血型特异性鉴定以及交叉配血试验。结果12例AIHA标本的Rh血型:氯喹放散处理前均为CcDEe,放散处理后CCDEE 2例、CcDEe 4例、ccDEE2例、ccDEe和CCDee各2例;抗体特异性:乙醚放散处理前无自身抗体,放散处理后血型特异性抗-E及抗-C各2例;选择无对应抗原的细胞,采用同型盐水法及抗球蛋白试验均血型相合。结论吸收放散能有效降低自身抗体对AIHA患者Rh血型鉴定和交叉配血的干扰,提高Rh血型鉴定和交叉配血的准确性。Objective To eliminate the interference of autoimmune hemolytic anemia(AIHA)on Rh blood group typing and antibody screening,so as to assure safe and effective transfusion for AIHA patients.Methods A cohort of 12 patients with AIHA submitted to Gao′an People′s Hospital from January 2019 to August 2019 were enrolled in this study.Rh blood group identification was conducted before and after chloroquine elution test,Specificity identification of autoantibodies and blood cross matching was carried out after ether elution test.Results The Rh blood types of 12 samples before chloroquine elution were all CcDEe but 2 cases of CCDEE,4 of CcDEe,2 of ccDEE,2 of ccDEe and 2 of CCDee were found after chloroquine elution.No specific autoantibody was detected before ether elution,.but 2 cases of specific anti-E and 2 cases of specific anti-C were found after ether elution.The E and C antigen negative red cells were selected for cross matching,and the results by physiological saline and anti-globulin methods demonstrated compatibility.Conclusion Absorption and elution test can effectively reduce the interference of autoantibodies on Rh blood group typing and blood cross matching in AIHA patients,and can improve the accuracy of blood group identification and blood cross matching.
