舌针结合“颅底七穴”治疗缺血性脑卒中后吞咽困难的疗效观察

摘　　要：目的:观察舌针结合"颅底七穴"治疗缺血性脑卒中后吞咽困难的临床疗效。方法:将72例缺血性脑卒中后吞咽困难患者随机分为治疗组和对照组,每组36例。治疗组采用舌针结合"颅底七穴"治疗,对照组予常规针刺治疗。两组均每日治疗1次,连续针刺6 d后休息1 d,7 d为1个疗程,共治疗2个疗程。治疗前后均以洼田吞咽能力评定法、吞咽困难分级量表和SSA评分进行疗效观察。结果:治疗后两组患者洼田吞咽能力评定法分级、吞咽困难分级量表评分、SSA评分均较治疗前改善(P<0.01),且治疗组各项指标均优于对照组(P<0.05);治疗组显效率为69.4%,高于对照组的33.3%(P<0.01)。结论:舌针结合"颅底七穴"可显著改善缺血性脑卒中后吞咽困难症状。Objective: To observe the clinical effect of tongue needling combined with "The Seven Acupoints of the Cranial Base" in the treatment of post-ischemic stroke dysphagia. Methods: Seventy-two patients who were in compliance with the inclusion criteria were randomly divided into a test group and a control group, 36 cases in each one. The test group was intervened by tongue needling combined with "The Seven Acupoints of the Cranial Base", while the control group was intervened by conventional acupuncture. The treatment was given once a day, rested for one day after continuous treatment for six days, seven days of treatment were taken as one session, continuously for two sessions. Before and after treatment, the curative effect was evaluated by Kubota’s swallowing ability assessment, Toshima Ichiro’s swallowing assessment, and Standardized Swallowing Assessment(SSA). Results: The grading of Kubota’s swallowing ability assessment, Toshima Ichiro’s swallowing assessment result, and SSA score were significantly improved in both groups after the treatment(P<0.01), and all indicators in the test group were better than those in the control group(P<0.05). The remarkably effective rate was69.4% in the test group, better than 33.3% in the control group(P<0.01). Conclusion: Tongue needling combined with "The Seven Acupoints of the Cranial Base" can significantly improve the post-ischemic stroke dysphagia.
