摘　　要：目的分析以头晕为主诉住院患者的常见病因,以期为临床头晕诊治提供一定的参考。方法分析2018-01—12在新密市中医院脑病科住院的所有以头晕为主诉符合入组标准的1014例患者的临床资料,根据病史、体征、发病特点,结合影像学、听力学、超声、前庭功能检测、精神心理测试等辅助检查,总结引起头晕的病因。结果1014例头晕患者病因分类良性阵发性位置性眩晕(BPPV)301例(29.68%),前庭性偏头痛185例(18.24%),精神疾患相关性眩晕166例(16.37%),后循环缺血114例(11.24%),梅尼埃病77例(7.59%),前庭神经炎46例(4.54%),其他28例(2.76%),原因不明97例(9.56%)。结论头晕最常见病因为良性阵发性位置性眩晕,与精神相关的眩晕多为慢性头晕患者,临床上应重视对慢性头晕患者的心理疏导及对患者基础疾病的管理和健康教育。Objective To analyze the common causes of hospitalized patients with dizziness as the main complaint,in order to provide a certain reference for the diagnosis and treatment of clinical dizziness.Methods The clinical data of 1014 patients with dizziness as the main complaint and met the inclusion criteria from January to December 2018 in the Encephalology Department of Xinmi Hospital of Traditional Chinese Medicine were analyzed.Auxiliary examinations such as science,ultrasound,vestibular function testing,and psychopsychological testing summarize the causes of dizziness.Results The etiology classification of 1014 dizziness patients301 cases(29.68%)with benign paroxysmal positional vertigo(BPPV),185 cases(18.24%)with vestibular migraine,and 166 cases(16.37%)with vertigo associated with mental disorders.There were 114 cases(11.24%)of circulatory ischemia,77 cases(7.59%)of Meniere's disease,46 cases(4.54%)of vestibular neuritis,and 28 other cases(2.76%),and 97 cases(9.56%)of unknown causes.Conclusion The most common cause of dizziness is benign paroxysmal positional vertigo.Mental-related vertigo are mostly chronic dizziness patients.Clinically,we should pay attention to the psychological counseling of patients with chronic dizziness,as well as the management and health education of patients with basic diseases.
