摘　　要：目的探究桥本甲状腺炎应用地塞米松局部免疫治疗的临床效果。方法78例桥本甲状腺炎患者,按照患者住院顺序分为实验组和对照组,各39例。对照组实施基础治疗,实验组在对照组基础上实施地塞米松局部免疫治疗。对比两组患者临床疗效、不良事件发生率及生活质量评分。结果实验组总有效率97.44%高于对照组的79.49%,差异有统计学意义(P<0.05)。实验组不良事件发生率2.56%低于对照组的20.51%,差异有统计学意义(P<0.05)。实验组日常生活评分为(94.25±2.36)分、健康状况评分为(94.36±1.25)分、总体精神评分为(96.32±1.12)分、活动能力评分为(89.36±1.25)分;对照组日常生活评分为(84.15±1.25)分、健康状况评分为(78.63±4.25)分、总体精神评分为(86.32±1.15)分、活动能力评分为(76.32±1.15)分。实验组日常生活、健康状况、总体精神、活动能力评分均高于对照组,差异有统计学意义(P<0.05)。结论桥本甲状腺炎患者通过地塞米松局部免疫治疗可以明显提高治疗效果,有效改善患者的生活质量,且不良事件发生情况较少,可在临床推广。Objective To investigate the clinical effect of local immunotherapy of Hashimoto’s thyroiditis with dexamethasone.Methods A total of 78 patients with Hashimoto’s thyroiditis were divided into experimental group and control group according to the order of hospitalization,with 39 cases in each group.The control group received basic therapy,and the experimental group received dexamethasone local immunotherapy on the basis of the control group.The clinical efficacy,incidence of adverse events and quality of life were compared between the two groups.Results The total effective rate 97.44%of the experimental group was higher than 79.49%of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse events 2.56%of the experimental group was lower than 20.51%of the control group,and the difference was statistically significant(P<0.05).In the experimental group,the daily life score was(94.25±2.36)points,the health status score was(94.36±1.25)points,the overall mental score was(96.32±1.12)points,and the activity ability score was(89.36±1.25)points;in the control group,the daily life score was(84.15±1.25)points,the health status score was(78.63±4.25)points,the overall mental score was(86.32±1.15)points,and the activity ability score was(76.32±1.15)points.The daily life score,health status,overall mental score and activity ability score of the experimental group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Dexamethasone local immunotherapy can significantly improve the therapeutic effect of patients with Hashimoto's thyroiditis,effectively improve the quality of life of the patients,and there are fewer adverse events,which can be promoted in the clinic.
