摘　　要：目的:探讨生物养育法对妊娠期糖尿病(gestational diabetes mellitus,GDM)剖宫产产妇母乳喂养的影响。方法:选取符合标准的150例GDM剖宫产产妇作为研究对象,采用电脑软件随机法分为对照组和试验组各75例。对照组予以传统母乳喂养,试验组采用生物养育法。比较两组产妇泌乳Ⅱ期启动时间、乳头疼痛评分、分娩后24 h新生儿有效吸吮数、6个月内纯母乳喂养率、产妇母乳喂养自信心及护理满意度等数据。结果:实际完成148例GDM剖宫产产妇的资料收集,两组各1例GDM剖宫产产妇退出研究。试验组产妇泌乳Ⅱ期启动时间短于对照组(P<0.05);试验组产妇乳头疼痛评分低于对照组(P<0.05);分娩后24 h新生儿有效吸吮数、6个月内纯母乳喂养率、产妇母乳喂养自信心及护理服务满意度评分高于对照组(P<0.05)。结论:生物养育法能有效缩短GDM剖宫产产妇泌乳Ⅱ期启动时间,提高6个月纯母乳喂养率,增强母乳喂养自信心,改善住院期间的就医体验。Objective:To explore the influence of biological nourishing on breastfeeding of cesarean women with gestational diabetes mellitus(GDM).Methods:A total of 150 pregnant women with GDM who met the research criteria were randomly divided into control group and experimental group by using computer software,75 cases in each.The cases in control group carried out traditional breastfeeding,and the cases in experimental group carried out biological nurturing.The start time of lactogenesis stageⅡ,the nipple pain scores,the times of newborn effective sucking during 24 h after delivery,the rate of exclusive breastfeeding within 6 months,the maternal self-confidence of breastfeeding and nursing satisfaction were compared between both groups.Results:The data collection of 148 cesarean women with GDM was actually completed.One case in each group withdrew from the study.The start time of lactogenesis stageⅡin the experimental group started earlier than that of the control group(P<0.05);the nipple pain score of patients in the experimental group was lower than that in the control group(P<0.05);the times of newborn effective sucking during 24 h after delivery,the rate of exclusive breastfeeding within 6 months,the maternal self-confidence of breastfeeding and nursing satisfaction were higher than those in control group(P<0.05).Conclusions:Biological nourishing can effectively shorten the start time of lactogenesis stageⅡof GDM cesarean women,increase the rate of exclusive breastfeeding within 6 months,strengthen breastfeeding self-confidence,improve the medical experience during hospitalization.
