高尿酸血症与冠状动脉支架置入术后再狭窄的关系探讨

摘　　要：目的:探讨高尿酸血症与冠状动脉支架置入术后支架内再狭窄的关系。方法:选取2014年9月—2019年3月间345例行经皮冠状动脉支架置入术的患者,根据血清尿酸水平分为两组,高尿酸组83例,正常尿酸组262组,在术后12~18个月复查冠脉造影,记录患者的冠心病危险因素及冠脉造影结果,支架内再狭窄的诊断标准为置入支架内或支架边缘5mm内血管内径狭窄大于或等于参照血管的50%,对结果进行分析。结果:高尿酸血症组共20例患者发生支架内再狭窄,正常尿酸组共29例发生支架内再狭窄,(24.1%vs 11.1%;P=0.003)。经logistic回归分析,校正年龄、BMI、冠心病相关病史、血脂、心功能及支架情况后,高尿酸血症仍是PCI术后ISR的独立危险因素(OR=2.673,95%CI:1.179~6.060)。结论:高尿酸血症与冠脉内支架再狭窄明显相关,降低血尿酸水平有助于减少再狭窄的发生。Objective To assess the relationship between hyperuricemia and intracoronary in-stent restenosis.Methods All the patients were selected from September 2014 to March 2019,totally 345 patients,all patinets received PCI,according to the serum uric acid level,divided into two groups:83cases in thehyperuric acid group and 262 cases in the normal uric acid group,after12 to 18months,all patients reviewed coronary angiography,record the coronary arterydisease risk factors and coronary angiography results.The diagnostic criterion ofin-stent restenosis is the stenosis of the inner diameter of the vessel greater than or equal to 50%of the reference vessel within 5 mm of the stent or the edge of the stent.Results In-stent restenosis occurred in 20 patients in the hyperuricemia group and 29 patients in the normal uric acid group(24.1%vs 11.1%;P=0.003).Logistic regression an-alysis show,:after adjusting for age,BMI,coronary artery disease related histor-y,blood lipid,cardiac function and st-ent status,hyperuricemia was still an independent risk factor for ISR after PCI(OR,95%ci=2.673,1.179~6.060).Conclusion Hyperuricemia is significantly ass-ociated with stent restenosis in coronary arteries.Lowering the uric acid level is helpful to reduce the occurrence of restenosis.
