健脾生血片并琥珀酸亚铁治疗缺铁性贫血患者的效果及对患者血清NO、BMP6水平的影响

摘　　要：目的探究健脾生血片并琥珀酸亚铁治疗缺铁性贫血患者的效果及对患者血清一氧化氮(NO)、骨形成蛋白6(BMP6)水平的影响。方法2016年6月—2019年6月我院确诊为缺血性贫血的患者153例,应用简单随机化分组方法分为对照A组、对照B组和观察组,每组各51例。对照A组予以健脾生血片治疗,对照B组予以琥珀酸亚铁治疗,观察组予以琥珀酸亚铁并健脾生血片治疗,3组均连续治疗8周。治疗结束比较3组临床疗效、不良反应发生率、治疗前后中医证候积分及全血血红蛋白(Hb)水平、红细胞(RBC)计数、红细胞平均体积(MCV),同时比较3组患者血清中铁(SI)水平、铁蛋白(SF)水平、NO水平、BMP6水平以及转铁蛋白饱和度(TSAT)。结果观察组临床总有效率优于对照A、B组(χ^2=8.053,P<0.05);疗程结束后1 d,观察组主要症状积分、全血红细胞参数(包括Hb、RBC、MCV)、血清铁代谢指标(包括SI、SF)、NO水平、BMP6水平与治疗前1 d的差值均高于对照A、B组的差值(F=4.796~550.888,P<0.05);3组不良反应总发生率比较,差异无显著意义(P>0.05)。结论健脾生血片并琥珀酸亚铁治疗缺铁性贫血,可明显减轻患者临床症状,改善红细胞参数及铁代谢指标,降低血清NO、BMP6水平,效果显著,且安全性高,值得临床推广与应用。Objective\To investigate the efficacy of Jianpi Shengxue tablets combined with ferrous succinate in the treatment of iron deficiency anemia and its effects on serum nitric oxide(NO)and bone morphogenetic protein 6(BMP6)levels.Methods\A total of 153 patients confirmed with ischemic anemia in our hospital from June 2016 to June 2019 were enrolled and divided into control group A(n=51),control group B(n=51),and observation group(n=51)using the simple randomized grou-ping method.The control group A was treated with Jianpi Shengxue tablets,the control group B was treated with ferrous succinate,and the observation group was treated with ferrous succinate combined with Jianpi Shengxue tablets.The three groups were treated for 8 consecutive weeks.After treatment,the three groups were compared in terms of clinical efficacy,incidence of adverse reactions,traditional Chinese medicine syndrome scores,whole blood hemoglobin(Hb)level,red blood cell(RBC)count,mean corpuscular volume(MCV),levels of serum iron(SI),serum ferritin(SF),NO,and BMP6,and transferrin saturation.\ResultsThe observation group had a significantly higher overall response rate than the two control groups(χ^2=8.053,P<0.05).Compared with the control groups A and B,the observation group had significantly greater changes in main symptom scores,whole blood erythrocyte parameters(including Hb,RBC,and MCV),serum iron metabolism parameters(including SI and SF),and le-vels of NO and BMP 6 from 1 d before treatment to 1 d after treatment(F=4.796-550.888,P<0.05).There was no significant difference in the overall incidence of adverse reactions between the three groups(P>0.05).\Conclusion\Jianpi Shengxue tablets combined with ferrous succinate in the treatment of iron deficiency anemia can significantly alleviate clinical symptoms,improve red blood cell parameters and iron metabolism parameters,and reduce serum NO and BMP6 levels,so this therapy has good efficacy and high safety and holds promise for clinical application.
