脓毒症患者血清中肾上腺髓质素原、和肽素表达水平及临床意义

摘　　要：目的:探讨脓毒症患者血清中肾上腺髓质素原(Pro-adrenomedullin,pro-ADM)及和肽素的表达水平及临床意义。方法:选取2018年10月-2019年9月南昌大学第一附属医院重症医学科收治的脓毒症患者94例,测定治疗前pro-ADM、和肽素表达水平,并记录患者急性生理学及慢性健康状况评分Ⅱ(Acute Physiological And Chronic Healthy EvaluationⅡ,APACHEⅡ)、序贯性器官功能衰竭评分(Sepsis-related Organ Failure Assessment,SOFA)。结果:脓毒症休克患者的pro-ADM、和肽素表达水平显著高于脓毒症患者,差异有统计学意义(P<0.01);死亡患者的pro-ADM、和肽素表达水平显著高于存活患者,差异有统计学意义(P<0.01);脓毒症患者pro-ADM、和肽素表达水平分别与APACHEⅡ评分和SOFA评分呈显著正相关。结论:pro-ADM、和肽素表达水平与脓毒症患者的危重程度及预后有关,可用于患者病情及预后的评估。Objective:To investigate the expression levels and clinical significance of pro-ADM and copeptin in the serum of patients with sepsis.Methods:The expression levels of pro-ADM and copeptin of 94 patients were measured before treatment,and the PACHEⅡand SOFA score were recorded.Results:The expression levels of pro-ADM and copeptin in patients with septic shock were significantly higher than those in patients with sepsis(P<0.01),and the expression levels of pro-ADM and copeptin in patients who died were significantly higher than survivors,with statistically significant differences(P<0.01).The expression levels of pro-ADM and copeptin in patients with sepsisu were significantly positively correlated with the APACHEⅡscore and SOFA score,respectively.Conclusion:The expression levels of pro-ADM and copeptin are related to the criticality of patients with sepsis,it can be used to evaluate condition and prognosis.
