银质针治疗中风恢复期肘关节痉挛临床疗效观察

摘　　要：目的:观察银质针与拮抗肌透刺法治疗中风后肘关节痉挛的临床疗效差异,明确银质针在中风恢复期肘关节痉挛的疗效优势。方法:采用随机数字表法将符合研究纳入标准的60例中风恢复期肘关节痉挛患者随机分对照组与治疗组,每组各30例。对照组采用拮抗肌透刺法治疗,治疗组采用银质针松解肘关节肱二头肌肌腱附近及肌腹的高张力点治疗。治疗期间两组患者均进行常规基础疾病控制及康复训练治疗。比较两组患者治疗前后上肢痉挛程度(MAS)评分、上肢运动功能量表(FMA)评分及日常生活活动能力量表(MBI)评分,并比较两组临床疗效。结果:治疗组临床疗效优于对照组(Z=-3.138,P=0.002<0.05);治疗后,两组患者FMA评分、MBI评分均较同组治疗前显著提高(P<0.05),且治疗组提高程度优于对照组(P<0.05);治疗后,两组患者MAS评分与治疗前比较均具有显著差异(P<0.05),且治疗组治疗后MAS评分降幅高于对照组(P<0.05)。结论:与拮抗肌透刺法比较,银质针松解肱二头肌肌腱附近及肌腹的高张力点法更能显著降低中风恢复期肘关节痉挛患者的上肢痉挛,改善上肢运动功能,提高日常生活活动能力,临床疗效更优。Objective:To observe the difference of clinical efficacy between silver acupuncture and antagonistic myotomyesis on elbow spasm after stroke,and to determine the advantage of silver acupuncture.Methods:60 patients were randomly divided into the control group and treatment group,30 cases in each group.The control group was treated by antagonistic muscle penetration,and the treatment group was treated by silver acupuncture to release the high tension points near the biceps tendon of the elbow and the muscle abdomen.Results:The clinical effect in the treatment group was better than the control group(Z=-3.138,P=0.002<0.05).FMA score and MBI score in groups were significantly higher than before treatment(P<0.05),and those in the treatment group were better than the control group(P<0.05).After treatment,there were significantly differences of MAS scores in groups compared with before treatment(P<0.05),and the decrease of MAS scores in the treatment group was higher than the control group(P<0.05).Conclusion:Compared with antagonistic muscle penetration,silver acupuncture can significantly reduce the upper limb spasm in patients with elbow joint spasm of stroke recovery,improve the upper limb motor function,improve the ability of daily life activity,and shows better clinical effects.
