加味五方散治疗活动期类风湿关节炎的临床观察

摘　　要：目的:观察加味五方散辅助治疗活动期类风湿关节炎(Rheumathoid Arthritis,RA)的临床疗效。方法:将60例活动期RA患者按随机数字表法分为治疗组和对照组各30例,对照组给予甲氨蝶呤、硫酸羟氯喹和美洛昔康口服治疗,治疗组在对照组基础上给予加味五方散外敷,观察治疗前后两组患者的关节疼痛评分、关节肿胀数、压痛数、晨僵时间及血常规、肝功能、C反应蛋白(C-reactive Protein,CRP),并计算治疗前后的28个关节疾病活动性评分(Disease Activity Score-28,DAS28)。结果:治疗组和对照组的总有效率均为93.3%(28/30),但治疗组患者临床显效和进步的例数明显多于对照组;治疗后患者关节肿胀数、压痛数、晨僵时间、疼痛数字分级法(Numeric Rating Scale,NRS)及DAS28均较前有明显好转,治疗组各项指标下降的幅度优于对照组(P<0.05),两组患者CRP均较前有降低,但组间比较降低幅度无统计学差异(P>0.05)。结论:在常规西药治疗的基础上联合加味五方散治疗活动期RA疗效好,可明显改善患者临床症状,降低RA的活动度。Objective:To observe the clinical effect of Wufang San(五方散)on active rheumatoid arthritis.Methods:60 active RA patients were divided into the treatment group and control group according to random numerical table.The control group was given methotrexate,hydroxychloroquine and meloxicam oral treatment,and the treatment group was given Wufang San more.Results:The total efficiency in both groups was 93.3%(28/30),but the number of clinical manifestations and progress in the treatment group were significantly higher than the control group.After treatment,the number of joint swelling,tenderness,morning stiffness time,NRS score and DAS28 score were significantly improved,and the reduction of various indicators was better than that in the control group(P<0.05),and the CRP of both groups were reduced,but there was no statistical difference in the reduction(P>0.05).Conclusion:Conventional Western medicine plus Wufang San on active RA Shows a good effect,can significantly improve the clinical symptoms,reduce RA activity.
