自拟活血祛瘀汤治疗冠状动脉粥样硬化性心脏病心绞痛的临床疗效观察

摘　　要：目的:探究活血祛瘀汤加减联合西医治疗冠状动脉粥样硬化性心脏病(Coronary Heart Disease,CHD,简称冠心病)心绞痛的临床疗效。方法:收集2018年12月-2019年6月期间我院心内病科治疗的患者共100例,采用随机数表法将其随机分成对照组和治疗组,分别50例患者。对照组患者进行常规的西医基础性治疗。治疗组患者在对照组治疗基础上予自拟活血祛瘀汤加减口服。分别对两组患者进行为期20天的进行性治疗。观察两组患者治疗前后患者中医证候积分(主症)、同型半胱氨酸(Homocysteine,Hcy)、超氧化物歧化酶(Superoxide Dismutase,SOD)、一氧化氮(Nitric Oxide,NO)、核转录因子-κB(Nuclera Factor-κB,NF-κB)含量及临床有效率。结果:两组的中医证候主症评分、Hcy、SOD、NO、NF-κB含量较治疗前均有所改善,且治疗后治疗组明显优于对照组,差异具有统计学意义(P<0.05)。治疗后对照组临床总有效率低于治疗组,差异具有统计学意义。结论:在西医基础上联合自拟活血祛瘀汤能够有效延缓心肌细胞损伤速度,改善血管通透性,降解脂类物质,改善血管痉挛,溶解血栓,抑制炎症反应发生,改善胸部针刺样疼痛、倦怠乏力、肢体沉重、喘憋气短等症状,提高临床有效率。Objective:To investigate the clinical efficacy of the Huoxue Quyu decoction(活血祛瘀汤)with Western medicine on angina pectoris in coronary heart disease.Methods:100 cases in our hospital were randomized into the control group and the treatment group,with 50 cases for each.The patients in the control group underwent routine Western medicine.The treatment group took the Huoxue Quyu decoction more.The treatment lasted for 20 days.Results:The scores of TCM syndrome,Hcy,SOD,NO and NF-κB in the groups were improved compared with before,and the above indicators in the treatment group were significantly better than the control group.The difference was statistically significant(P<0.05).After treatment,the total efficiency in the control group was lower than the treatment group,and the difference was statistically significant.Conclusion:On the basis of Western medicine,the Huoxue Quyu decoction on angina pectoris in coronary heart disease can effectively delay the rate of myocardial cell injury,improve vascular permeability,degrade lipids,ameliorate vasospasm,dissolve thrombus,inhibit inflammation and ameliorate symptoms such as lancinating pain in the chest,fatigue,heavy limbs,shortness of breath,and improve clinical efficacy.
