肛肠病术后尿潴留患者行五苓散对总有效率的价值研究

摘　　要：目的:将五苓散应用于肛肠术后尿潴留患者,观察其治疗效果。方法:本次研究选择我院2018年3月-2019年3月期间收治的68例肛肠病术后尿潴留患者,根据治疗性质随机分组,其中对照组33例(甲基硫酸新斯的明),治疗组35例(五苓散加减方),对比两种治疗方法的有效率,不良反应,视觉模拟评分法(Visual Analogue Scale,VAS)疼痛评分,治疗后中医证候积分,治疗后尿常规、尿培养及尿道分泌物培养阳性率,以及并发症发生率。结果:治疗组有效率高于对照组,不良反应发生率低于对照组(P<0.01);治疗后治疗组与对照组的VAS评分均有明显的改善,治疗组的评分明显低于对照组(P<0.01);治疗组治疗后的中医证候积分明显低于对照组(P<0.01);治疗组治疗后的尿常规阳性率、尿培养阳性率及尿道分泌物培养阳性率均明显低于对照组(P<0.05);治疗组发生尿路感染、肾积水、膀胱破裂等并发症的概率低于对照组(P<0.05)。结论:将五苓散应用于肛肠病术后尿潴留病症的治疗中有利于提高患者病症治疗效果,缩短临床症状缓解时间,改善排尿疼痛感,促进实验室检查结果转阴,降低不良并发症发生机率,且该治疗方案安全性较高,值得推广。Objective:Wuling San(五苓散)was applied in patients with urinary retention after anorectal surgery,and its therapeutic effect was observed.Methods:68 cases were randomly divided into groups according to the nature of treatment,namely 33 patients in the control group(took neostigmine methyl sulfate)and 35 cases in the treatment group(received Wuling San).Results:The tatal efficiency in the treatment group was higher than that in the control group,and the incidence of adverse reactions was lower than the control group(P<0.01).VAS scores in the groups were both significantly improved after treatment,and the scores in the treatment group were significantly lower than the control group(P<0.01).TCM syndrome scores in the treatment group after treatment were significantly lower than those in the control group(P<0.01).The positive rate of urine routine,urine culture and urethral secretion culture positive rate after treatment in the treatment group were significantly lower the control group(P<0.05).The incidence of complications such as urinary tract infection,hydronephrosis,and bladder rupture in the treatment group was lower than the control group(P<0.05).Conclusion:Wuling San on urinary retention after anorectal surgery is beneficial to improve clinical effect,can shorten the time of clinical symptom relief,ameliorate the pain of urination,promote the negative result of laboratory examination,and reduce the incidence of adverse complications;the treatment plan is safe and worthy of promotion.
