小青龙汤加附子治疗腰椎间盘突出导致神经根水肿的疗效研究

摘　　要：目的:探讨小青龙汤加附子用于治疗腰椎间盘突出导致神经根水肿的临床疗效。方法:将200例腰间盘突出导致神经根水肿的患者随机分成观察组和对照组。对照组应用甘露醇、地塞米松联合治疗,丹参注射液(活血药物)进行辅助治疗,观察组仅单独应用小青龙汤加附子治疗,3周后分析临床疗效情况。结果:治疗前,两组患者的Oswestry功能障碍指数(Oswestry Dysfunction Index,ODI)相比,差异无统计学意义(P>0.05);治疗后,观察组的ODI明显低于对照组,差异有统计学意义(P<0.05)。观察组疗效显著高于对照组(P<0.05)。两组患者的疼痛程度均有所减轻,疼痛评分均有所下降,观察组的评分下降程度明显高于对照组,差异有统计学意义(P<0.05);治疗前,将两组患者的日本骨科协会评估治疗分数(Japanese Orthopaedic Association Scores,JOA)下腰痛评分相比,差异无统计学意义(P>0.05);在治疗后,将两组患者的JOA下腰痛评分相比,观察组的JOA下腰痛评分明显优于对照组,差异有统计学意义(P<0.05)。结论:小青龙汤加附子治疗腰椎间盘突出症引起的神经根水肿具有较好效果。Objective:To study the efficacy of the Xiaoqinglong decoction(小青龙汤)plus aconite on nerve root edema caused by lumbar disc herniation.Methods:200 patients with lumbar disc herniation leading to nerve root edema were randomized into the observation group and the control group.The control group was treated with mannitol,dexamethasone and salvia injection for adjuvant treatment.The observation group was treated with the Xiaoqinglong decoction plus aconite.The clinical efficacy was analyzed after 3 weeks.Results:Before treatment,the Oswestry dysfunction index(ODI)in the two groups was no statistically significant difference(P>0.05).After treatment,the ODI in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).The efficiency in the control group was lower than in the observation group,P<0.05.The pain degree and pain score in the two groups were decreased,and the observation group was significantly higher than that in the control group(P<0.05).Before treatment,the Japanese orthopaedic association scores(JOA)in the two groups were no statistically significant difference(P>0.05).After treatment,the JOA low back pain scores in the observation group were significantly better than those in the control group(P<0.05).Conclusion:The Xiaoqinglong decoction has a good effect on the treatment of nerve root edema caused by lumbar disc herniation.
