宫外孕方合脱花煎加减在药物终止妊娠中应用的临床观察

摘　　要：目的:探讨药物终止妊娠中应用宫外孕II号方合脱花煎后的临床效果。方法:将120例需要药物终止妊娠的患者分为两组:治疗组(60例)和对照组(60例)。两组患者均服用米非司酮片25~50 mg,1日2次,连服2~3 d,总量150~200 mg。第3~4天清晨口服米索前列醇片600μg或阴道后穹窿放置卡前列甲酯栓1枚,根据排胎情况必要时加服米索前列醇或阴道再次放置卡孕栓。服用米非司酮后,治疗组服用中国中医药“自拟宫外孕II号方合脱花煎”,对照组不接受任何其他药物治疗。结果:治疗组阴道平均流血时间为(8.14±2.24)d,排出绒毛胎囊时间为(3.21±0.68)h,完全流产45例,不完全流产15例,继续妊娠0例,完全流产率为75.00%(45/60),米索前列醇或卡孕栓用药1次;对照组平均流血时间为(11.25±2.85)h,排出绒毛胎囊时间为(6.29±1.35)h,完全流产率为31例,不完全流产23例,继续妊娠6例,完全流产率为51.67%(31/60)。米索前列醇或卡孕栓用药2~3次,两组比较,治疗组疗效优于对照组,排出绒毛胎囊时间、阴道出血量和出血明显少于对照组。结论:药物流产后,结合服用中药宫外孕II号方合脱花煎,阴道流血量减少、流血和排出绒毛胎囊时间减短,并提高药物流产成功率。Objective:To investigate the clinical effect of prescription combined with Tuohua decoction(脱花煎)of ectopic pregnancy II in drug termination of pregnancy.Methods:120 patients who needed medication to terminate their pregnancy were divided into two groups:the treatment group(60 cases)and the control group(60 cases).Patients in both groups were given mifepristone tablets 25-50 mg twice a day for 2-3 days,with a total dose of 150-200 mg.In the morning of the 3rd to 4th day,oral misoprostol tablet 600ug or one capoprostol suppositor was placed in the posterior fornix of the vagina.If necessary,misoprostol or vaginal suppositor was added.After taking mifepristone,the treatment group received the traditional Chinese medicine"prescription combined with Tuohua decoction of ectopic pregnancy II,while the control group did not receive any other drug treatment.Results:The mean vaginal bleeding time in the treatment group was(8.14±2.24)days,and the time of discharging the tomentous sac was(3.21±0.68)hours.There were 45 cases of complete abortion,15 cases of incomplete abortion,and 0 case of continued pregnancy.The rate of total abortion was 75.00%(45/60),misoprostol or calprogesterone was used once.In the control group,the mean bleeding time was(11.25±2.85)days,and the time of discharging the tomentous sac was(6.29±1.35)hours.There were 31 cases of complete abortion,23 cases of incomplete abortion,and 6 cases of continued pregnancy.The rate of total abortion was 51.67%(31/60),misoprostol or cargestational suppository was used for 2-3 times.Compared with the control group,the curative effect of the treatment group was better.The discharge time,vaginal bleeding and bleeding of the treatment group were significantly less than those of the control group.Conclusion:After medical abortion,the use of prescription combined with Tuohua decoction of ectopic pregnancy II can reduce vaginal bleeding volume,blood and discharge of villus fetal sac time and improve the success rate of medical abortion.
