摘  要:目的:探讨耐碳青霉烯类鲍曼不动杆菌耐药性与adeB基因的关系,为临床治疗提供新思路。方法:收集2018年1月至2019年1月肺部感染患者痰液标本642份,分离鉴定鲍曼不动杆菌株,采用PCR检测其adeB基因表达,采用微量二倍稀释法检测MIC50。结果:分离出鲍曼不动杆菌250株,adeB基因表达异常的为152株。adeB异常表达组中亚胺培南耐药87株(57.24%)、美罗培南耐药79株(51.97%)均高于adeB正常表达组。治疗后adeB异常表达组亚胺培南、美罗培南MIC50均显著低于治疗前(P<0.05)。结论:adeB基因异常的鲍曼不动杆菌对碳青霉烯类抗菌药物更易产生耐药,加入外排泵抑制剂后能提高碳青霉烯类抗菌药物的治疗效果。Objective: To investigate the relationship between adeB gene and drug resistance of carbapenem resistant Acinetobacter baumannii,so as to provide new ideas for clinical treatment.Methods: 642 sputum samples from patients with pulmonary infection from January 2018 to January 2019 were collected for the isolation and identification of Acinetobacter baumannii strains.The adeB gene expression of Acinetobacter baumannii strains was detected by PCR, and MIC50 was detected by microdilution method.Results: 250 strains of Acinetobacter baumannii were isolated and 152 strains were abnormal in adeB gene expression.87 strains( 57.24%) were resistant to imipenem and 79 strains( 51.97%) were resistant to meropenem in abnormal adeB group,which were all higher that those in normal adeB group.After treatment,the lowest MIC50 values of imipenem and meropenem in adeB abnormal group were significantly lower than those before treatment( P < 0.05).Conclusion: Acinetobacter baumannii with abnormal adeB gene is more resistant to carbapenems.The efflux pump inhibitor can effectively improve the therapeutic effect of carbapenems.
