涤痰汤加减联合依达拉奉注射液治疗高血压脑出血临床疗效及对脑功能的保护作用

摘　　要：目的研究探讨涤痰汤加减联合依达拉奉注射液对高血压脑出血的治疗效果及脑功能的保护作用。方法选取2018年8月—2019年12月医院住院治疗的88例高血压脑出血患者为研究对象,按随机抽样的方式分为观察组、对照组,各44例。对照组给予常规治疗及依达拉奉治疗,观察组在对照组基础上联合涤痰汤加减治疗。对两组患者的临床疗效、神经功能评分、脑功能指标、不良反应发生率等进行统计比较。结果观察组总有效率为90.91%(40/44),高于对照组为72.73%(32/44)(P <0.05);治疗后观察组神经功能缺损评分(NIHSS)低于对照组,格拉斯哥昏迷指数评分(GCS)、蒙特利尔认知功能评分(MoCA)均高于对照组(P <0.05);治疗后观察组胶质纤维酸性蛋白(GFAP)、细胞间黏附因子-1(ICAM-1)、神经元特异性烯醇化酶(NSE)水平均低于对照组(P <0.05);治疗期间观察组、对照组不良反应发生率分别为13.64%(6/44)、6.82%(3/44),组间比较无显著差异(P> 0.05)。结论涤痰汤加减联合依达拉奉注射液对高血压脑出血有良好的治疗效果,能改善神经功能及认知功能评分,降低各项脑损伤标志物水平,治疗安全性良好,可临床推荐。Objective To study the therapeutic effect of Ditan decoction Combined with edaravone injection on hypertensive cerebral hemorrhage and the protection of brain function. Methods 88 patients with hypertensive intracerebral hemorrhage who were treated in our hospital from August 2018 to December 2019 were collected as the study objects. According to the way of random sampling,they were divided into observation group and control group,44 cases each. The control group was treated with edaravone and the observation group with Ditan decoction. The clinical effect,neurological score,brain function index and adverse reaction rate of the two groups were compared. Results the total effective rate was 90. 91% (40/44) in the observation group,which was higher than 72. 73% (32/44) in the control group (P < 0. 05). After treatment,NIHSS in the observation group was lower than that in the control group,GCS and MOCA were higher than those in the control group (P < 0. 05).The levels of GFAP,ICAM-1 and NSE in the observation group were lower than those in the control group (P < 0. 05). During the treatment period,the incidence of adverse reactions in the observation group and the control group were 13. 64% (6/44) and 6. 82% (3/44),respectively. There was no significant difference between the two groups (P > 0. 05). Conclusion-Ditan decoction Combined with edaravone injection has a good therapeutic effect on hypertensive intracerebral hemorrhage. It can improve the cognitive function score of neurological function,reduce the level of brain injury markers,and has a good therapeutic safety,which can be recommended clinically.
