北京市医耗联动综合改革对某三甲公立医院医疗收入的影响分析

摘　　要：[目的]分析北京市医耗联动综合改革对某三甲公立医院医疗收入及其结构变化等造成的影响,并提出有效应对措施。[方法]以某三甲公立医院为例,提取该院2019年下半年的医疗服务量、医疗收入、典型病例费用相关数据,并与2018年同期结果进行对比分析。[结果] 2019年下半年(改革后)该医院门急诊人次较2018年同期(改革前)下降0.95%,出院人次下降3.94%,手术人次上升21.03%;医疗收入增长1.84%,其中,化验收入下降1.64%,卫生材料收入下降1.42%,治疗收入提升4.55%,手术收入提升72.57%,患者次均门急诊费用上升2.39%,例均出院费用上升6.47%。[结论]医耗联动综合改革有助于进一步理顺医疗服务比价关系和体现医务人员劳动价值,推动医院规范诊疗行为、提升医疗水平、控制医药费用不合理增长。Objective To analyze the influence of that medical consumables comprehensive reform give rise to medical income and its structural changes in a tertiary public hospital in Beijing,and puts forward effective solutions.Methods It took tertiary public hospital as an example,the hospital's medical service volume,medical income,and typical case costs data in second half of 2019 in that hospital were extracted and compared with the results of the same period in 2018.Results In second half of 2019(after the reform),hospital outpatient and emergency department visits decreased by 0.95%compared with the same period in 2018,person-time of discharges decreased by 3.94%,person-time of surgical visits increased by 21.03%,medical income increased by 1.84%,of which,test income decreased by 1.64%,health materials income decreased by 1.42%,treatment income increased by 4.55%,surgical income increased by 72.57%,patients'average person-time of outpatient and emergency expenses increased by 2.39%,and patients'average per-case discharge cost increased by 6.47%.Conclusions The medical consumables comprehensive reform helps to further rationalize price relationship of medical service and reflect the labor value of medical personnel,promote hospitals to standardize diagnosis and treatment behaviors,improve medical services level,and control unreasonable growth of medical expenses.
