术前体位训练联合视频健康教育对经胆道镜T管窦道取石术患者围术期应激反应的影响

摘　　要：目的:研究视频健康教育结合术前体位训练对经胆道镜T管窦道取石术患者生理应激的影响。方法:选取经胆道镜T管窦道取石术患者105例,随机分为对照组52例和观察组53例,对照组患者术前进行视频健康教育,观察组患者在对照组的视频健康教育基础上增加术前体位训练,对比两组患者术后心室起搏管理功能(MVP)、心率(HR)、呼吸频率(RR)、术后VAS评分效果。结果:观察组患者术中HR为(87.56±5.03)次/min,低于对照组患者的(99.47±6.72)次/min(t=9.234,P=0.000);观察组患者术后HR为(83.21±4.73)次/min,低于对照组患者的(92.13±5.65)次/min(t=7.883,P=0.000);观察组患者术中、术后RR、MVP均低于对照组(P<0.05),观察组患者术后1 d、3 d末VAS评分均低于对照组(P<0.05)。结论:视频健康教育结合术前体位训练应用于经胆道镜T管窦道取石术患者,可降低患者的平均动脉压、心率、呼吸次数,缓解疼痛。Objective:To study the effect of video health education combined with preoperative posture training on the physiological stress of patients undergoing T-tube sinus stone extraction via choledochoscope.Methods:A total of 105 patients undergoing choledochoscope T-tube sinus stone removal were randomly divided into 52 cases in the control group and 53 cases in the observation group.The control group received video health education before the operation,and the observation group received video health education in the control group.The postoperative postoperative position training was added to compare the postoperative ventricular pacing management function(MVP),heart rate(HR),respiratory rate(RR)of the two groups of patients,and the effect of VAS score at the end of the first and third days after surgery.Results:The intraoperative HR(87.56±5.03)times/min of the observation group was lower than that of the control group(99.47±6.72)times/min,which was statistically significant(t=9.234,P=0.000);postoperative HR(83.21±4.73)times/min min is lower than that of the control group(92.13±5.65)times/min,which is statistically significant(t=7.883,P=0.000);intraoperative and postoperative RR and MVP of the observation group are lower than those of the control group,1,3 days after operation.The final VAS score was lower than that of the control group,which was statistically significant(P<0.05).Conclusion:Video health education combined with preoperative posture training applied to patients undergoing choledochoscope T-tube sinus lithotripsy can reduce the patients'average arterial pressure,heart rate,breathing frequency,and relieve pain.
