腹腔镜下脾切除术中静脉通道麻醉复合局麻对手术麻醉效果、应激指标影响的临床研究

摘　　要：目的:分析腹腔镜下脾切除术中静脉通道麻醉复合局麻的应用效果。方法:将80例行腹腔镜下脾切除术患者按抽签法进行分组,静脉通道组40例,静脉通道麻醉;复合组40例,静脉通道麻醉复合局麻;评估麻醉效果,比较其麻醉应激反应。结果:两组拔管、苏醒及手术时间比较差异无统计学意义(P>0.05)。复合组手术结束、术后6 h应激指标水平升高程度低于静脉通道组,术后6、12及24 h血浆S100β含量升高程度低于静脉通道组(P<0.05)。复合组麻醉不良反应占5.0%,静脉通道组占12.50%,两组比较差异无统计学意义(P>0.05)。结论:在腹腔镜下脾切除术中采用静脉通道麻醉结合复合局麻,麻醉效果好,应激反应轻。Objective:To analyze the effect of intravenous anesthesia combined with local anesthesia during laparoscopic splenectomy.Methods:80 patients underwent laparoscopic splenectomy were divided into two groups according to the method of drawing lots,40 cases in venous channel group(intravenous channel anesthesia),and 40 cases in compound group(intravenous channel anesthesia combined with local anesthesia).The anesthetic effect was evaluated and the anesthetic stress reactions were compared.Results:The time of extubation,recovery and operation of the two groups were not different significantly(P>0.05).The adverse reactions of anesthesia in the composite group accounted for 5.0%,and the intravenous access group accounted for 12.50%,and there was no significant difference between the two groups(P>0.05).Conclusion:Intravenous anesthesia combined with local anesthesia during laparoscopic splenectomy has good anesthetic effect and light stress response.
