原络调神针刺法对脑卒中后抑郁患者血清IGF-1、BDNF、NGF水平及神经功能的影响

摘　　要：目的:探讨原络调神针刺法对脑卒中后抑郁患者血清IGF-1、BDNF、NGF水平及神经功能的影响。方法:选取2018年3月—2020年3月我院收治的80例脑卒中后抑郁患者,采用随机数字表法分为研究组和对照组,各40例。对照组给予盐酸氟西汀胶囊治疗,研究组在对照组的基础上给予原络调神针刺法治疗,两组均连续治疗8周。比较两组治疗8周后的临床疗效;对比两组治疗前、治疗8周后的抑郁程度、神经功能缺损程度、生活质量、血清炎症因子及神经细胞因子水平。结果:治疗8周后,研究组总有效率为95.00%(38/40),高于对照组的72.50%(29/40),组间比较差异具有统计学意义(P<0.05)。与治疗前比较,治疗8周后两组HAMD、MESSS评分及血清IL-6、IL-1β和TNF-α水平均降低,且治疗8周后,研究组均低于对照组(P<0.05);两组ADL评分及血清IGF-1、BDNF和NGF水平均升高,且治疗8周后,研究组均高于对照组(P<0.05)。结论:采用原络调神针刺法治疗脑卒中后抑郁,可明显升高患者血清IGF1、BDNF和NGF水平,改善神经功能,同时可减轻机体炎症反应,进而有助于提高临床疗效。Objective:To investigate the effects of Yuanluo Tiaoshen acupuncture on serum levels of IGF-1,BDNF and NGF and its influence to neurological function in patients with post-stroke depression.Methods:80 patients with post-stroke depression were randomly divided into the the study group and the control group,with 40 cases in each group.The control group was treated with Fluoxetine Hydrochloride Capsules;on which basis,the study group was also treated with Yuanluo Tiaoshen acupuncture.Both groups were treated conti-nuously for 8 weeks.The clinical efficacy was compared between the two groups after 8 weeks of the treatment.The depression level,neurological deficit level,quality of life,the levels of serum inflammatory factors and nerve cell factors were compared between the two groups before and after 8 weeks of the treatment.Results:After 8 weeks of the treatment,the total effective rate was 95.00%(38/40)in the study group,which was significantly higher than 72.50%(29/40)in the control group(P<0.05).The scores of HAMD,MESSS and the serum levels of IL-6,IL-1βand TNF-αwere significantly decreased after the treatment compared to those before the treatment in the two groups;of which the decreases in the study group were more significant(P<0.05).The score of ADL and the serum levels of IGF-1,BDNF and NGF were significantly increased after 8 weeks of the treatment compared to those before the treatment in the two groups,and the increases in the study group were more significant(P<0.05).Conclusion:Yuanluo Tiaoshen acupuncture can significantly increase the serum levels of IGF1,BDNF,and NGF,improve neurological function and reduce the body's inflammatory response to improve clinical efficacy in the treatment of post-stroke depression.
