泻阴补阳针刺法联合针刺“五泉穴”治疗卒中后痉挛性瘫痪的疗效及对血清TGF-β1和NSE水平影响

摘　　要：目的:探讨泻阴补阳针刺法联合针刺“五泉穴”治疗卒中后痉挛性瘫痪的疗效及对血清TGF-β1和NSE水平影响。方法:选取我院收治的卒中后痉挛性瘫痪患者86例,随机分为两组各43例。对照组给予针刺“五泉穴”,治疗组在对照组基础上给予泻阴补阳针刺法,对比两组治疗前后的临床疗效、上肢和下肢Ashworth评分、Barthel评分、CSI评分、FIM评分和FMA评分及血清TGF-β1和NSE水平。结果:经过治疗后,治疗组总有效率较高(P<0.05);两组患者上肢和下肢Ashworth评分明显降低(P<0.05),且治疗组降低较多(P<0.05);两组患者Barthel评分、FIM评分和FMA评分水平显著升高,CSI评分明显降低(P<0.05),且治疗组改善较多(P<0.05);两组患者NSE水平显著降低,TGF-β1水平显著升高(P<0.05),且治疗组改善程度较大(P<0.05)。结论:采用泻阴补阳针刺法联合针刺“五泉穴”治疗卒中后痉挛性瘫痪具有较好的治疗效果,能够改善血清TGF-β1、NSE水平,安全性较好,值得在临床上推广应用。Objective:To investigate the efficacy of Xieyin Buyang acupuncture combined with needling Wuquan points in the treatment of post-stroke spastic paralysis and its effect on serum levels of TGF-β1 and NSE.Methods:86 patients with post-stroke spastic paralysis were randomly divided into the control group and the treatment group,with 43 cases in each group.The control group was treated with needling Wuquan points;on which basis,the treatment group was also treated with Xieyin Buyang acupuncture.The clinical efficacy,the scores of Ashworth,Barthel,CSI,FIM and FMA of the upper and lower limbs,as well as the serum levels of TGF-β1 and NSE were compared between the two groups after the treatment.Results:The total effective rate was higher in the treatment group than that in the control group(P<0.05).Ashworth scores of upper and lower limbs were significantly decreased in the two groups after the treatment(P<0.05);the decreases in the treatment group were more significant(P<0.05).The scores of Barthel,FIM and FMA were significantly increased,and CSI scores were significantly decreased in the two groups after the treatment(P<0.05);of which the improvements in the treatment group were more significant(P<0.05).The level of NSE was significantly decreased,and the level of TGF-β1 was significantly increased in the two groups after the treatment(P<0.05);of which the changes in the treatment group were more significant(P<0.05).Conclusion:Xieyin Buyang acupuncture combined with needling Wuquan points has a good therapeutic effect on post-stroke spastic paralysis,it can improve the serum levels of TGF-β1 and NSE with its high safety.The therapy is worthy of clinical promotion and application.
