摘　　要：目的:通过系列举措对实施门诊实名全预约分时段就医的推动作用与成效。方法:2020年2月20日开始宁波市第一医院采取系列举措,修订制度与应急预案,多途径宣传,运用信息化完善当天预约功能,院内志愿者指导门诊患者预约、取号,收集问题及时整改等。结果:门诊预约就诊率、按时就医率明显递增;门诊患者退号减少;门诊患者对预约就医理念提升,优化就医流程合理分流患者,达到分时段有序就医。结论:门诊就诊模式由随意就诊转向医患双方主动预约就诊的模式,全预约分时段就医可比较合理分流患者,减少患者在院内等候时间;医院需要进一步简化预约流程,对部分老年人预约就医需要发挥社区医师作用,规范分级诊疗行为,改善患者就医体验。Objective:Impact and effectiveness of a series of initiatives for the implementation of the real-name system for time-based outpatient visits all by appointment.Methods:Starting from February 20,2020,Ningbo First Hospital has adopted a series of measures to revise the system and emergency plan,promote through multiple channels,and use information technology to improve the appointment function on the same day.Volunteers in the hospital guide outpatients to make appointments and get registered,and collect questions in a timely manner rectification and so on.Results:The rate of outpatient appointments and on-time medical visits increased significantly;outpatient withdrawal numbers decreased;outpatients’concept of making appointments for medical treatment improved,optimizing medical procedures to rationally divert patients and achieving orderly visits in time intervals.Conclusion:The mode of outpatient visits has changed from random visits to a mode where both doctors and patients take the initiative to make appointments.All-appointed visits at different times can divert patients reasonably and reduce the waiting time in the hospital.The hospital needs to further simplify the appointment process and make appointments for some elderly Give full play to the role of community physicians,standardize hierarchical diagnosis and treatment behavior,and improve patients'experience of seeking medical care.
