摘 要:目的建立动态比浊法检测血浆凝固酶方法并进行能力评估。方法以PCR检测nuc基因为参考,分析比较动态比浊法、试管法检测血浆凝固酶的特异性、敏感性及检测时限的差别,并比较其与VITEK 2 Compact鉴定结果的差异。结果共计收集的200株葡萄球菌,其中金黄色葡萄球菌100株,非金黄色葡萄球菌100株,动态比浊法检测发现98株金黄色葡萄球菌血浆凝固酶阳性,2株血浆凝固酶阴性,而100株非金黄色葡萄球菌血浆凝固酶均为阴性,特异度、敏感度分别为100%和98%。动态比浊法检测血浆凝固酶所需时间更短,2 h检测敏感性达83%,而试管法仅53%,两者差异有统计学意义(P <0.01)。结论动态比浊法或可代替试管法进行血浆凝固酶的检测,可更加准确快速地鉴别金黄色葡萄球菌。Objective The purpose of this study is to establish a dynamic turbidimetric method for the determination of plasma coagulase and to evaluate its capability.Methods PCR was used to detect nuc gene and set as reference,the differences in specificity,sensitivity and detection time limit of plasma coagulase were analyzed and compared between dynamic turbidimetric method and test-tube method,and the differences in identification results of plasma coagulase and VITEK 2 Compact were also compared.Results A total of 200 strains of Staphylococcus aureus were collected,including 100 strains of Staphylococcus aureus and 100 strains of non-Staphylococcus aureus.Dynamic turbidimetric assay showed that 98 Staphylococcus aureus plasma coagulases were positive,2 were negative,and 100 non-Staphylococcus aureus plasma coagulases were negative,with specificity and sensitivity of 100% and 98%.The dynamic turbidimetric method took shorter time to detect plasma coagulase,and the sensitivity reached 83% after 2 h,while the test tube method was only 53%,and the difference was statistically significant( P <0.01).Conclusion Dynamic turbidimetric method can be used to detect coagulase in plasma instead of test tube method,which can identify Staphylococcus aureus more accurately and quickly.
