77例湿疹患者的自身抗体谱研究

摘　　要：目的通过对77例湿疹患者血清中抗核抗体谱检测结果的分析,了解湿疹患者的自身抗体分布特点,探讨抗核抗体谱在湿疹患者中的临床意义。方法获取77例湿疹患者3 ml外周血,提取血清并使用欧盟公司免疫荧光方法以及免疫印迹法对77例湿疹患者血清进行抗核抗体谱检测,荧光显微镜下观察免疫荧光结果,半定量免疫印迹仪对抗核抗体中自身抗体谱进行半定量检测并记录结果。结果在77例湿疹患者血清中,14例患者抗核抗体弱阳性(1∶32),5例患者抗核抗体滴度阳性(1∶100),6例患者抗核抗体滴度强阳性(1∶320),其中U1RNP阳性1例,SSA阳性5例,Ro抗体阳性8例,SSB阳性3例,PM-SCL阳性2例,组蛋白抗体阳性1例,核糖体P蛋白阳性2例,着丝点抗体阳性3例,线粒体抗体阳性3例,抗核膜抗体阳性1例,其中湿疹患者合并3例干燥综合征及2例系统性红斑狼疮。结论 32.4%湿疹患者自身抗体异常,且会合并干燥综合征或系统性红斑狼疮,临床诊疗时应予以重视。Objective To understand the distribution characteristics of autoantibodies in 77 eczema patients and to explore the clinical significance of autoantibody spectrum in eczema patients.Methods A total of 3 ml of peripheral blood were collected from 77 eczema patients.After 3000 rpm centrifugation for 15 minutes and anticoagulated with ordinary biochemical tube,the serum was collected and the autoantibody spectrum of 77 eczema patients were detected by immunofluorescence method and Western blotting method.The results of immunofluorescence were observed under fluorescence microscope.The semi-quantitative immunoblotting instrument was used to detect the autoantibody.The autoantibody spectrum of the eczeme patients were semi-quantitatively detected and the results were recorded.Results Among 77 eczema patients,14 patients were weak positive for autoantibody( 1∶32),5 cases were positive for autoantibody( 1∶100),6 cases were strong positive for autoantibody( 1∶320),and the autoantibody spectrum including 1 U1 RNP positive,5 SSA positive,8 Ro positive,3 SSB positive,2 PM-SCL positive and 1 histone antibody positive,2 ribosomal P protein cases,3 centromere antibody cases,3 mitochondrial antibody cases and 1 anti-nuclear membrane antibody case.And 3 patients of eczema complicated with Sjogren’s syndrome and 2 patients with systemic lupus erythematosus were observed.Conclusion 32.4% of eczema patients have abnormal autoantibodies and may be complicated with Sjogren’s syndrome or systemic lupus erythematosus.Attention should be paid to clinical diagnosis and treatment.
