摘　　要：目的分析总结烧伤患者创面受细菌沾染(感染)情况,为临床预防用药选择及治疗提供经验。方法通过回顾性研究2015年3月-2018年2月在本科住院的576例烧伤患者创面分泌物培养结果和对临床同期所用抗生素敏感率结果进行统计分析。结果3年内共作创面分泌物培养576例,其中阳性410例(71.2%),阴性166例(28.8%)。检出阳性菌17种~20种,共883株,其中单菌种120例(29.3%),混合菌种291例(70.7%)。菌种以球菌为主(40.88%),其中金黄色葡萄球菌占21.52%;各种杆菌占29.21%;大肠杆菌属占6.56%;假单胞菌占13.70%;链球菌属占6.79%。临床常用的丁胺卡那霉素、氧氟沙星、头孢呋辛等均有60%以上的敏感率。结论在烧伤创面感染的防治中,应注重及时清创和手术切除坏死组织,并及时选用以针对球菌为主,兼顾杆菌的抗生素。Objective To analyze and summarize the bacterial contamination( infection) on the wound of burn patients,and provide basis for the selection and treatment of clinical prophylactics.Methods A retrospective study was conducted to analyze the culture results of wound secretions from 576 burn patients admitted to the hospital from March 2015 to February 2018 and the sensitivity rate of antibiotics used in the clinical period.Results A total of 576 cases of wound secretions were cultured within3 years,among which 410 cases were positive( 71.2%) and 166 cases were negative( 28.8%).17-20 strains of positive bacteria were detected,including a total of 883 strains,including 120 single strains( 29.3%) and 291 mixed strains( 70.7%).The main strains were Cocci( 40.88%),Staphylococcus aureus( 21.52%).Bacilli accounted for 29.21%;Escherichia coli accounted for 6.56%;Pseudomonas accounted for 13.70%;Streptococcus accounted for 6.79%.The sensitivity of amikacin,ofloxacin,cefuroxime and other commonly used drugs was more than 60%.Conclusion In the prevention and treatment of burn wound infection,attention should be paid to timely debridement and surgical resection of necrotic tissue,and timely selection of antibiotics mainly targeting Cocci and taking into account Bacillus.
