多普勒超声和CT联合检查对甲状腺乳头状癌及颈部淋巴结转移的临床诊断价值

摘　　要：目的分析超声联合计算机断层扫描(CT)检查在诊断甲状腺乳头状癌及颈部淋巴结转移的应用价值。方法选取80例疑似甲状腺乳头状癌的患者为研究对象,均行CT及超声检查。以术后病理检查结果为诊断金标准,分析单独超声检查、单独CT检查、CT联合超声检查的准确率、敏感度及特异度,并分析3种检查方法在诊断颈部淋巴结转移中的效果。结果CT检查、超声检查及联合检查诊断准确率分别67.50%、66.25%、90.00%。CT及超声检查总符合率比较,差异无统计学意义(P>0.05);联合诊断总符合率高于CT、超声单独检查,差异有统计学意(P<0.05)。结论CT联合超声检查诊断甲状腺乳头状癌的效果较好,淋巴结转移诊断符合率也较高。Objective To analyze the clinical value of ultrasound combined with computed tomography(CT)examination in diagnosis of patients with papillary thyroid cancer and cervical lymph node metastasis.Methods A total of 80 patients with suspected thyroid papillary carcinoma were selected as study objects,CT and ultrasound examinations were performed for all patients.Taking results of postoperative pathological examination as gold standard in diagnosis,the accuracy,sensitivity and specificity of ultrasound examination,CT examination as well as CT and ultrasound examination in combination were analyzed.The effects of the three examination methods in diagnosis of cervical lymph node metastasis were analyzed.Results The diagnostic accuracy was 67.50%in CT examination,66.25%in ultrasound examination,and 90.00%in combined examination.The total coincidence rate between CT and ultrasound examination showed no significant difference(P>0.05).The total coincidence rate of combined diagnosis was significantly higher than that of CT and ultrasound examination alone(P<0.05).Conclusion CT and ultrasound examination in combination has a higher efficacy in diagnosis of papillary thyroid carcinoma,and a higher coincidence rate in diagnosis of lymph node metastasis.
