摘  要:目的观察实时聚合酶链反应(PCR)检测在妊娠晚期B族链球菌(GBS)筛查诊断中的应用价值,并分析GBS阳性孕妇不良妊娠结局情况。方法将我院917例妊娠晚期孕妇作为研究对象,所有孕妇均采用实时PCR技术和细菌培养法进行GBS筛查,对比两种方法阳性率,并比较GBS阳性和GBS阴性孕妇不良妊娠结局发生率。结果细菌培养显示GBS阳性者38例,实时PCR检测显示GBS阳性者96例。实时PCR诊断妊娠晚期GBS阳性的敏感度、特异度和准确度分别为100%、99.52%和99.56%。GBS阳性组胎膜早破、宫内感染、胎儿窘迫和新生儿感染发生率均高于GBS阴性组(P<0.05)。结论实时PCR检测对妊娠晚期GBS筛查诊断价值较高,妊娠晚期GBS感染会增加不良妊娠结局发生率。Objective To observe application value of real-time polymerase chain reaction(PCR)detection in screening and diagnosis of group B streptococcus(GBS)in late pregnancy,and to analyze adverse pregnancy outcomes of positive GBS pregnant women.Methods A total of 917 pregnant women in late pregnancy in the hospital were enrolled as study objects.The real-time PCR and bacterial culture were applied to screen GBS.The positive rates of the two methods were compared.The incidence of adverse pregnancy outcomes between GBS-positive and GBS-negative pregnant women was compared.Results Bacterial culture showed there were 38 cases(4.14%)with positive GBS.Real-time PCR showed that there were 96 cases(10.47%)with positive GBS.The sensitivity,specificity and accuracy of real-time PCR for diagnosis of positive GBS in late pregnancy were 100%,99.52% and 99.56%,respectively.The incidence of premature rupture of membranes,intrauterine infection,fetal distress and neonatal infection in positive GBS group was higher than that in negative GBS group(P <0.05).Conclusion Real-time PCR detection is of higher screening and diagnosis value for GBS in late pregnancy.GBS infection in late pregnancy will increase incidence of adverse pregnancy outcomes.
