摘  要:目的基于德尔菲法构建口腔刺激、非营养性吸吮及口腔支持三者联合使用的综合口腔运动干预方案,并探讨其对早产儿喂养进程及生长发育的影响。方法邀请28名省内外新生儿领域的专家进行两轮德尔菲法专家咨询,根据专家的意见和建议修改、完善、形成最终版综合口腔运动干预方案。选取144例早产儿为研究对象,随机分为综合口腔运动干预组(IOMIs组)、口腔运动干预组(OMIs组)和对照组,每组各48例。对照组实施早产儿常规发育支持护理,在对照组的基础上,IOMIs组实施综合口腔运动干预,OMIs组实施Fucile口腔运动干预。观察记录早产儿开始经口喂养时间和完全经口喂养时间,每天监测体重1次,入院及入院第14天,检测早产儿血清瘦素、胰岛素样生长因子-1的表达水平,并进行比较。结果 IOMIs组和OMIs组早产儿喂养进程均小于对照组(P<0.05)。IOMIs组和OMIs组早产儿喂养进程无统计学差异(P>0.05)。IOMIs组恢复出生体重的时间小于OMIs组和对照组(P<0.05)。入院第14天时,IOMIs组和OMIs组早产儿胰岛素样生长因子-1和瘦素的水平高于对照组(P<0.05)。结论综合口腔运动干预不仅可节约时间,还可缩短早产儿的喂养进程和恢复出生体重的时间,提高血清瘦素和胰岛素样生长因子-1的水平,促进早产儿的生长发育,值得在临床推广使用。Objective To construct the integrated oral motor interventions by combining Hwang’s 5 minoral motor interventions and Boiron’s 2 min oral support together,based on the Delphi method,and explore the effects on oral feeding ability and growth and development of preterm infants.Methods Invited 28 experts in the field of neonatology around China andconducted two rounds of expert consultation by Delphi method,then revised and improved the integrated oral motor intervention program according to the opinions and suggestions of experts,and formed the final version.a total of 144 preterm infants hospitalized in neonatal intensive care were selected as participants,they were randomly divided into integrated oral motor interventions group(IOMIs group),oral motor interventions group(OMIs group)and the control group according to the hospitalization order,each group had 48 cases participants.Preterm infants in the control group received the developmental supportive care,in addition to the developmental supportive care,the IOMIs group also received integrated oral motor interventions,the OMIs group also received Fucileoral motor interventions.Observed and recorded the beginning of oral feeding and the full of oral feeding,measured body weightonce a day.On the admissionand the 14th day of admission,measured the expression level of leptin andinsulin-likegrowthfactor-1,Then compared.Results The feeding process of IOMIs group and OMIs group were all less than that of the control group(P<0.05).There was no significant difference of feeding process between IOMIs group and OMIs group(P>0.05).The time reached to birth weight of IOMIs group was shorter than the OMIs group andcontrol group(P<0.05).On the 14th day after admission,the levels of leptin and insulin-like growth factor-1 of preterm infants in IOMIs group and OMIs group were higher than those of the control group(P<0.05).Conclusion Integrated oral motor interventions could shorter the feeding process and time reached to birth weight of preterm infants,improve the level of l
