摘  要:为了探讨体外阿米卡星(AK)与头孢噻呋(EFT)、红霉素(E)以及马波沙星(MBF)单独及联合应用对奶牛乳房链球菌耐药突变选择窗(MSW)的影响,试验对临床采集的奶样进行乳房链球菌的分离鉴定,采用微量肉汤稀释法和棋盘法测定AK、EFT、E、MBF单用及联用后对分离的乳房链球菌的最小抑菌浓度(MIC),计算部分抑菌浓度(FIC)指数,判断药物联用后的相互作用;采用琼脂二倍稀释法测定药物单用及联用后对乳房链球菌的的防耐药突变浓度(MPC),计算选择指数SI(MPC/MIC),判定能否缩小MSW,筛选出最佳联合用药方案。结果表明:分离得到7株乳房链球菌,单用时,AK、EFT、E、MBF对分离的7株乳房链球菌的MIC分别为8~64、0.5~8、0.125~16、1~16μg/mL;联用后,AK与EFT、E、MBF的FIC指数分别为0.5~1、0.3125~0.75、0.3125~0.75。单用时,AK、EFT、E、MBF对分离的7株乳房链球菌的MPC分别为128~512、64~512、16~256、64~512μg/mL;AK与EFT、E、MBF联用后,SI分别为原来的50%~100%、25%~100%、12.5%~100%。说明AK与其他三种药物联用后,对乳房链球菌均具有协同或相加作用,且AK与其他三种药物联用后均可以缩小MSW,其中AK与MBF联用后效果较为明显。To investigate the effects of amikacin(AK)alone and in combination with ceftiofu(EFT),erythromycin(E)and marbofloxacin(MBF)on the mutation selection window(MSW)against Streptococcus uberis from dairy cows,Streptococcus uberis was isolated and identified from clinical milk samples,the MIC of AK,EFT,E and MBF alone or in combination of Streptococcus uberis isolated from dairy cows was determined by the micro broth dilution method and the checkerboard method,fractional inhibitory concentration(FIC)index was calculated to check the interaction of the drug combination.The agar double-dilution method was used to determine the mutant prevention concentration(MPC)both alone and combined against Streptococcus uberis and the selection index SI(MPC/MIC)was calculated to determine whether the drug resistance mutation selection window(MSW)can be narrowed and screen out the best combination medication plan.The results showed that 7 strains of Streptococcus uberis were isolated,when used alone,the MIC of AK,EFT,E,MBF were 8-64,0.5-8,0.125-16,1-16μg/mL,respectively;After combined use,the FIC index of synergistic the combination were 0.5-1,0.3125-0.75,0.3125-0.75,respectively;When the drug was used alone,the MPC of AK,EFT,E,and MBF to the 7 isolated Streptococcus uberis were 128-512,64-512,16-256,64-512μg/mL,respectively;After AK was combined with EFT,E,and MBF,SI was 50%~100%,25%~100%,12.5%~100%of the original respectively.It showed that AK had a synergistic or additive effect on Streptococcus uberis after combined with three other drugs,and AK could reduce MSW after combined with other three drugs,and AK combined with MBF had a better effect obvious.
