噻托溴铵联合布地奈德/福莫特罗治疗支气管色素沉着纤维化的疗效观察

摘　　要：目的:评价吸入噻托溴铵联合布地奈德/福莫特罗松在改善支气管色素沉着纤维化(bronchial anthracofibrosis,BAF)肺功能及临床症状方面的作用。方法:收集120例BAF患者随机分成三组,噻托溴铵组单独吸入噻托溴铵,布地奈德/福莫特罗组单独吸入布地奈德/福莫特罗,联合吸入组联合吸入噻托溴铵与布地奈德/福莫特罗,共治疗12周,分别于治疗的第4周、第8周、第12周记录患者的如下指标:(1)肺功能(FEV1%、FVC%);(2)改良的医学研究学会呼吸困难量表(MMRC)评分;(3)6 min步行距离(6MWD);(4)观察三组的不良反应。结果:三组肺功能指标FEV1%、FVC%、MMRC、6MWD较治疗前明显改善,差异有统计学意义(P<0.05),单独吸入噻托溴铵组及单独吸入布地奈德/福莫特罗组第4周肺功能:FEV1%、FVC%、MMRC、6MWD较治疗前显著改善,差异有统计学意义(P<0.05),但第8周、第12周各指标与自身第4周比较,差异无统计学意义(P>0.05);联合吸入组第4周FEV1%、FVC%、MMRC、6MWD较治疗前显著改善,差异有统计学意义(P<0.05),但第8周、第12周与自身第4周比较,FEV1%、FVC%差异无统计学意义(P>0.05),联合吸入组第8周6MWD、MMRC评分较自身第4周比较,差异有统计学意义(P<0.05),但第12周6MWD、MMRC评分与自身第8周比较,差异无统计学意义(P>0.05);单独吸入噻托溴铵组与单独吸入布地奈德/福莫特罗组相同治疗时期各指标间,差异无统计学意义(P>0.05),但联合吸入组相同治疗时期上述指标改善程度均优于单独吸入噻托溴铵组、单独吸入布地奈德/福莫特罗组,差异有统计学意义(P<0.05)。结论:支气管色素沉着纤维化目前尚无统一的治疗方法,单独吸入噻托溴铵及布地奈德/福莫特罗均对其有效,但联合吸入两种药物疗效更显著。Objective To evaluate the effect of inhaled tiotropium and budesonide/formoterol on lung function and clinical symptoms of bronchial anthracofibrosis(BAF).Method A total of 120 patients with BAF were randomly divided into 3 groups.Tiotropium bromide group received inhaled tiotropium bromide alone.budesonide/formoterol group received inhaled budesonide/formoterol alone.Combined inhalation group received inhaled tiotropium and budesonide/formoterol,placebo inhaled for 12 weeks in the control group,The patients were recorded respectively on the 4 th,8 th and 12 th week of treatment,the indicators were as follows:(1)Pulmonary function,modified(FEV1%、FVC%);(2)To improve the medical research council dyspnea scale(MMRC);(3)6 min walking distance(6 MWD);(4)the adverse events of the three groups were observed.Results Three groups indexes of pulmonary function:FEV1%、FVC%,MMRC and 6 MWD were significantly improved before treatment(P<0.05),The lung function in inhaled tiotropium group and inhaled budesonide/formoterol group in the 4 th week were significantly improved before treatment(FEV1%,FVC%,MMRC,6 MWD)(P<0.05).However,no significant difference was found between the 8 th,12 th and the 4 th week oneself(P>0.05).In the fourth week,the lung function(FEV1%、FVC%),MMRC and 6 MWD of combined inhalation group were significantly improved compared with those before treatment(P<0.05).However,FEV1%、FVC%no significant difference was found between the 8 th,12 th and the 4 th week oneself(P>0.05).Inhalation of tiotropium and budesonide/formoterol group MMRC and 6 MWD were significantly improved in the 8 th week compared with the 4 th week(P<0.05),however the 12 th week compared with the 8 th week,there was no significant difference between each index(P>0.05).There was no significant difference between tiotropium group and budesonide/formoterol group during the same treatment period(P>0.05),However,the improvement rate of these indexes in the same treatment period in the combined inhalation group was better than those in the tio
