小剂量肝素在儿童脓毒症早期中的应用疗效分析

摘　　要：目的:探讨小剂量肝素在儿童脓毒症早期中应用的临床疗效。方法:选取2016年1月~2019年12月70例脓毒症患儿为研究对象,按不同治疗方式分为对照组与研究组,每组各35例。入院后全部患儿均给予病因治疗、脏器支持等综合治疗,研究组加用小剂量肝素辅助治疗,比较两组患儿危重症评分(PCIS)、D-二聚体及PLT水平,并观察凝血功能指标变化及并发症发生率。结果:与对照组相比,研究组PCIS评分、PLT水平显著升高,D-二聚体水平显著下降,差异有统计学意义(P<0.05);同时研究组PT、APTT明显缩短,MODS、DIC的发生率显著降低,差异有统计学意义(P<0.05),两组不良反应发生率差异无统计学意义(P>0.05)。结论:在儿童脓毒症早期应用小剂量肝素治疗,可显著改善凝血功能,降低DIC、MODS的发生率,改善预后,且安全性高。Objective To investigate the clinical effect of low-dose heparin in early childhood sepsis.Method From January 2016 to December 2019,70 cases of early childhood sepsis were involved.According to different treatment methods,70 cases were divided into control group and study group,35 cases in each group.All the patients were treated by comprehensive treatment such as etiology treatment and organ support.Of which,the study group were treated with comprehensive treatment plus low-dose heparin.Then,children critical disease score(PCIS),D-dimer and PLT levels were compared,and changes of coagulation indexes and incidence of complications were observed.Results Compared with the control group,PCIs and PLT levels of the study group were significantly increased,D-dimer levels significantly decreased(P<0.05).Meanwhile,PT and APTT were significantly shortened,incidence of MODS and DIC significantly reduced(P<0.05).However,there were no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Application of low-dose heparin in early childhood sepsis can significantly improve coagulation function,reduce incidence of DIC and MODS,improve prognosis and safety.
